Job summary This is an exciting opportunity for an experienced Advanced Nurse Practitioner to develop their clinical practice within a large teaching, training and research ANP will act as an autonomous practitioner within a team of clinicians in a GP surgery, managing a broad range of clinical consultations with diagnosis, assessment and treatment of patients.
You will have your own surgeries to see a range of patients within your competencies and also dedicated sessions to manage the nursing home residents and review and support our dementia patients with the support of the GP frailty and dementia lead.You will work with a team of ANPs and have monthly mentoring from a named GP to support and develop your practice.
Additionally you will have the opportunity to attend regular clinical team meetings for clinical updating, safeguarding ,significant events and journal club.
Main duties of the job The successful candidate will demonstrate excellent communication skills, flexibility, and reliability with an organised approach to work.In this role you will work independently, making professional, autonomous decisions.The role will involve initial history taking, clinical assessment, diagnosis, treatment plan and evaluation of patient care.
You will support the GP team managing both acute and routine presentations of a wide range of clinical problems .
You will have your own surgery list and also undertake home visits and attend patients in the nursing home.You will be responsible for making any necessary referrals, requesting investigations, managing your results and letters in an appropriate manner and timescale.The role will involve independent prescribing within the scope of professional practice.The nurse practitioner will need to prioritise and triage the needs of patients accordingly, seeking GP input as appropriate where the problem lies outside their professional capability.You will demonstrate safe clinical decision making and a high level of independent care including assessment, diagnostic and prescribing skills.We are supportive of and encourage learning and development, providing in house monthly mentoring opportunities and regular team training and educational sessions.
In addition to your 30 days (pro rate) annual leave you will receive 5 days Study leave each year (pro rata ) to enable protected time for your professional development.
About us Currently our multi-disciplinary team includes an administration and reception team, 13 GPs, 3 Advanced Nurse Practitioners and an experienced nursing team.
We are a supportive practice with an ethos of teaching and training .We work from a purpose built health centre set in the beautiful woodland of Nightingale Valley where we enjoy team walks and a large garden to use for our breaks.As part of Connexus PCN we employ clinical pharmacists, social prescribers, First Contact Physios, Paramedic to deliver services that support a wide range of patients and assist with the clinical workload.
Job description Job responsibilities SUMMARY OF MAIN RESPONSIBILITIESTo work as an autonomous practitionerResponsible for:Attending to patients who wish to be seen by a health care professionalDiagnosing, treating and prescribing for patients presenting with undifferentiated and undiagnosed illness or injuryMaking an assessment of their needs, based on highly developed nursing skills including physical assessment.Seeing and advising people in respect of their continuing medical and nursing needsPatients are to be seen in a timely manner enabling contractual requirements to be metThe ANP will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.
There will be a requirement to see extra or emergency patientsResponsible for developing, with the patient, a continuing management plan for the health of the patient and when appropriate to refer to other clinicians and agenciesTo provide high quality nursing services to a defined practice populationThe role is flexible and demanding and may be expanded in accordance with practice requirements and the nurse practitioners own scope of practice.
Person Specification Qualifications Essential Experience in a similar role Desirable Relevant nursing/health degree Other relevant post registration qualifications Experience Essential Minimum 5 years post registration experience At least 2 years recent primary and community nursing experience Nurse-led management of minor illness, minor ailments and injuries Nurse-led triage Compiling protocols and clinical guidelines Leadership in quality initiatives such as clinical benchmarking Audit Research Nurse prescribing Skills Essential Demonstrable experience of managing minor illness Management of patients with complex needs Clinical examination skills Good understanding of local and national health policy Clinical governance issues in primary care Clinical leadership skills Communication skills, both written and verbal Communication of difficult messages to patients and families Desirable Teaching and mentorship clinical setting Resource management Knowledge of public health issues Able to identify determinants on health in the area Management experience
