Job summary We are seeking to appoint a dynamic, enthusiastic and highly motivated, forward thinking Band 6 Clinical Nurse Specialist, to work within the Specialist Palliative Care team.
The post is part of a team of specialist palliative care nurses covering the hospital and community seven days a week including bank holidays.
This service is part of the Integrated Community Services Division who provide a multitude of services within Bolton.
Experience of working in community is not essential as the successful candidate will receive a full supportive induction when they join the Division and the team.

Main duties of the job The post holder will be a proven team player with excellent communication skills with significant, relevant experience.
They will contribute to the provision of specialist palliative and end of life care to the people of Bolton with advanced life shortening disease, working autonomously and as part of a team.
They will deliver clinical interventions across care settings providing advice, education and support to patients, their carers and other health care professionals.
They will also promote advance care planning, management of complex end of life symptoms and patient and carer expectations whilst meeting the integrated care needs of the patient and preventing inappropriate admissions to hospital.
In addition, they will be able to contribute to the development of services, teaching, audit and research.
The individual will be supported to develop their competencies in this specialty through a programme of induction, education and mentorship, tailored to their level of experience and expertise.Specialist Palliative Care services have been developed, over the years, in an integrated and collaborative manner between the hospital, community, and Bolton Hospice, reflecting excellent co-operation between the organisations and a patient centred whole-team approach.

Job description Job responsibilities The post holder is responsible for the assessment and implementation of Specialist Palliative Care and Advice.
They manage a patient caseload and act as a link between primary, secondary and tertiary care.
They implement and deliver specialist training and education programmes to staff and students.
They work independently with support from the Palliative Care Team.
They practice in the development of protocols and undertake and evaluate research and clinical audit.
They will contribute to the provision of a 7-day service with on-site weekend and bank holiday working.New Health and Social Care Act Regulations were approved by Parliament on the 22nd July 2021, which requires all those working or entering a CQC registered care home (for persons who require nursing or personal care) to be fully vaccinated (Covid 19) from the 11th November 2021, unless exempt under the regulations.
You will therefore be required to provide proof of vaccination or exemption if you are successful at interview.
