The post holder will be responsible for:oTo be responsible for the provision of Occupational Therapy in our Rehabilitation Beds ensuringthat services support the discharge of patients by providing specialist assessment and work as part of a multidisciplinary team in the rehabilitation units to facilitate timely assessmentand timely, co-ordinated discharge planning as requiredoTo work within professional standards and clinical guidelines, promote best practice and toundertake all aspects of clinical duties as an autonomous practitioner including assessing andtreating own caseload of patients/clients and maintaining professional documentation.
Main duties of the job To maintain communication links with other disciplines and agencies by attending operational meetings and through verbal and written recording and reporting.
To maintain effective communication by attending necessary multidisciplinary team meetings.
To communicate effectively with patients and carers to maximise rehabilitation potential and to ensure safe discharge planning.
To advise and instruct relatives, carers and other professionals as appropriate.
To maintain concise and accurate reports following all departmental standards and procedures.

To complete OT specific assessments including those required to assess cognition, sensory, visual and postural needs, including planning and carrying out suitable treatment plans.
To independently manage a delegated caseload of straightforward and complex cases demonstrating a high level of problem solving, clinical reasoning skills and professional judgement.
To select and apply specific Occupational Therapy assessments addressing occupational performance and skills deficits enabling clients in the areas of productivity, self-maintenance and leisure.
To carry out and record comprehensive assessments within the ward/ therapy department and home environment.
To prescribe and monitor equipment, adaptations and assistive technology to improve or maintain independence.
Person Specification Education/Qualification Essential Diploma/degree in Occupational Therapy Current UK AHP HCPC registration Desirable Post graduate degree in relevant area Experience Essential Post registration as an occupational therapist working with older adults, falls prevention Supervising staff/students Working in a multi-disciplinary team Desirable Experience working in a wide range of specialist services eg.
falls services, wheelchair services, neurology, and orthopaedics Leading appraisals Experience of implementing evidence-based care and developing others to do so.
