What the Job Involves Facilitate supported self-assessments and promote informed decision-making among service users, grounded in understanding of the Care Act and safeguarding practice.
Conduct assessments involving risk, need, and capacity, ensuring that service users can identify and access services both short and long-term, including the use of Telecare.
Liaise effectively with intra and interagency teams, working towards joint service delivery and case management.
Oversee financial aspects, ensuring compliance with Slough Borough Council policies and maintaining high-quality data records.
Engage in safeguarding activities, respond to complaints, and contribute to departmental practice improvement through systematic evaluation and auditing.
Work to prevent hospital admissions and support residents during or post-hospital stay by fostering strength-based conversations and interventions.
Ensure accurate data management on the electronic LAS system and review care plans as per policy guidelines to promote safe and effective community and workplace practices.
Work twice a week from the office as part of a duty function ensuring the completion of safeguarding responsibilities and CHC-related tasks.
