Scale 29 - £38,626 to Scale 34 - £43,693 with a professional bar at SCP31 £40,476** Hours Per Week: 36 hours Base Location: Tameside Integrated Care Foundation Trust, Fountain Street, Ashton Under Lyne, OL6 9RW Assessment Date : To be arranged The Role This is an exciting opportunity to come and work for Tameside Adult's Services on the Hospital Social Work team, based at Tameside General Hospital.

About You You will take the lead on supporting adults to live well at home.
You will be part of a friendly and supportive team and your work will include managing a caseload, Care Act Assessments, Support Plans, Mental Capacity Assessments and Best Interest Decisions and signposting to internal and external services where necessary to enhance wellbeing and independence.
You will have excellent communication skills and have the ability to build effective professional relationships with other disciplines and with other local health and social care services to enable a coordinated response to individuals and families.

Experience of working with adults and previous employment in a similar position would be desirable.
It is essential that you hold a current valid driving licence and have access to a vehicle for work use.

Relevant professional qualification for the job role at Level 6 or above, AND 2.
Minimum of 2 years post qualification experience in the role, AND 3.

Professional standards practised to the highest expected level.
