Job summary We are seeking a compassionate and dynamic new member of our multi-professional team to provide social work support to the Inpatient Unit, Community Team and Day Services.

Main duties of the job Overviewof Role:To work as a member of the multi professional team providing socialwork support to the Inpatient Unit, community team and Day Services includingsupport for relatives and carers of Mountbattens patients.To work with patients and families in a way that identifies their personal goalsand to help tailor care from Mountbatten and external organisations to supportthese goals.To support patient and their families within their own socialcontext and cultural frameworks and advocate for them as and when required.To act as thesafeguarding lead and enable the wider MDT to identify and respond tosafeguarding concerns.
To deliver regular safeguarding training as part of thelead safeguarding role.Undertake holisticassessments for patients, cares, service users and ensure they are reviewed.

Undertakingassessment of need considering current support systems and including potentialrisks, for example Mental Capacity Act, continuing care and safeguardingissues.Support patients and their families in the development of support plansusing a person-centred approach in liaison with statutory and non-statutory /voluntary agencies.
Through using the wide range of tools and options availableto support the service user in determining their own outcomes and how theycould be met.Enable patients choice and empowerment through promoting independence asfar as possible.
Using knowledge and skills, to promote quality of life,through the application of the principles of the personalisation agenda, suchas support planning and risk enablement.To facilitate patients/carers in the identification of services within thecommunity and purchasing of services on their behalf to meet both their shortterm and long-term needs.To ensure ahigh level of data quality for all information recorded, ensuring that theinformation is timely, accurate and complete.To maintain and contribute to the development of relevant recordsand statistical data upholding information governance.To hold acaseload of a level of complexity and provide case management, guidance,support and expert advice to the multi-disciplinary team as and when required.To assess andmanage risk to individuals, families, carers, groups, communities, self andcolleagues.Understand and respond to ethnic,cultural and religious influences of the patient and family / carers /friends.Encourage, develop and maintain effective working relationships withservice colleagues and those in a wide range of partner agencies and services,locally and nationally.To work as a member of multi-disciplinaryteams providing expert opinions on the issues around theCare Act (2014) and Social CareTo present patients at multidisciplinarymeetings from individual caseload and where required on behalf of the team.To negotiate with a range of local authorityservices, community health care teams, local service providers, voluntary organisations andagencies to meet the needs of patients being discharged from the MountbattenIn-Patient UnitTo consult and work effectively with otherprofessionals to identify and meet patient / carers needs, positivelycontributing to the achievement of joint working practices.Have a good working knowledge of theContinuing Health Care process and eligibility criteria to support dischargeplanning.Promote multi-disciplinary workingpartnerships, sometimes acting as a liaison between the Council and otherhealth and social care partners.To carry out assessment and dischargeplanning in accordance with the Care Act (2014) with patients admitted toMountbatten IPU unit and source continuing care and support, working closelywith Hampshire and Southampton.Professional Registration and StandardsKeep up to date with training and personal development to ensureprofessional registration with the Social Work England must be maintained.
Take responsibility for ensuring professional practice isdelivered within the boundaries of the Codes of Practice for Social Workers.Keep up to date with relevant legislation or changes in socialpolicy that impact on social work practice.To manage and prioritise workloads carrying out all duties usingaccountable professional judgment and knowledge base on social work practices.Audit, Research and EducationProvide learning opportunities for student social workers.
Participate in education and provide advice to, for example,social work and health colleagues.Adopt a reflective approach from personal practice and learn fromexperienceAct as educational resource To have a clear understanding and working knowledge of both adultand childrens safeguarding policies and procedures.To research and maintain a knowledge base of what resources areavailable at a local and national level to inform decision making processes.To actively participate in the development of bespoke educationalprogrammes of teaching to the MDT and external courses as required tobereavement and loss, safeguarding and social care.To be aware and keep updated on changes at national, regional, andlocal legislation, theories, policies and practices in order to disseminateknowledge throughout the team and contribute to the delivery of a high standardof service.To lead on and participate in service improvement projects asrequired.To engage in audit, evaluation and research as required.To monitor own performance against agreed objectives throughMountbatten's appraisal systems.
To provide training as part of the training programme for staffand volunteers including safeguarding and mental capacity/DOLSCommunicationEffectively communicate with patients, carers, nursing staff,allied health professionals, volunteers, the wider multi-professional team,community and hospital teams and other primary care services.To demonstrate advanced communication skills in order that complexand emotionally sensitive situation are managed in an appropriate way.Respond to feedback as appropriate from patients, families, carersand colleaguesEnsure effective record keeping and documentation.Promote an open and honest culture Person Specification Other Essential Willing to undertake training Current Driving Licence and use of a vehicle, with insurance for business purposes Experience Essential Knowledge of Social Work theory, practice and values Post qualifying experience Understanding of relevant legislation/guidance including Care Act, Mental Capacity Act / DOLS and Safeguarding Skills Essential Effective time management skills, with ability to prioritise/adapt to the unpredictable nature of the work Abilities Essential Commitment to and enthusiasm for multi-disciplinary and interagency working Willing to work flexibly to meet the demands of the service Ability to deal with people and situations sensitively and appropriately Qualifications Essential CQSW or Diploma in Social Work or BA (Hons) Social Work Current registration with the Health and Care Professions Council
