The successful candidate will work as part of a supportive team to provide assessments of need, care and support planning, and review, that takes a strengths-based approach, with individuals who may have a range of social care needs, their families, and informal carers.

We are looking for a passionate, enthusiastic Social Worker to support patients to be discharged from hospital under the D2A model.
You will be qualified to degree level and be SW England registered.
Your main duties will be to provide an assessment, support planning and review service, underpinned by professional knowledge and legislation in order to ensure that: Complex individuals are supported appropriately including undertaking mental capacity assessments and best interest processes.
Vulnerable adults are safeguarded and are supported to live well with risk.
Strength based conversations that result in person centred outcomes and support requirements are identified, and.
People are encouraged and enabled to live healthy and independent lives.

Key Responsibilities: To prepare for social work contact and involvement, including reviewing case notes, liaising with others for additional information and evaluating information.
To undertake the assessment of support needs for Service Users, Carers and Families, where their circumstances may be difficult and unstable, and assist in screening referrals.
To develop/ assist to develop Care & Support Plans for Service Users.
To produce, deliver, evaluate, and update Care & Support Plans and packages, in order to resolve practical and emotional social care, health and well-being difficulties.
To be proactive with respect to Safeguarding issues and following relevant policies and procedures if abuse is suspected.
To identify, assess and manage risks to Service Users, Carers and Families, and contribute towards a positive change to behaviour to reduce risk.
To achieve the objectives of the Council and to contribute to the development and implementation of the Annual Team Plan and assist to achieve the objectives of the team, reviewing the progress of the team against targets and contribute to guidance, policies, and practice notes.
To assist to develop, implement and monitor quality assurance standards.

Excellent communication and engagement skills are required to effectively advise and support service users and their Carers It is essential you are able to understand the principles of safeguarding or be prepared to undertake the appropriate training.
The ability to work in a multi-disciplinary team to deliver social work services.
Excellent communication and interpersonal skills and the ability to work with confidence to challenge where appropriate and based on evidence.
