Main duties of the job To appraise the CAMHS Clinical Nurse Specialist and MDT of the young person's progress and observations of changes in their mental health condition and ensure these changes are documented on the electronic record• To have a close working relationship with the CAMHS Clinical Nurse Specialist and nominated care co-ordinators• To regularly meet with the supervisor and manager to ensure the post holder is supported to carry out the demands of the post• To ensure that all care services provided are offered without prejudice regardless of age, creed, gender and ethnicity• To be aware of diverse cultural and ethnic needs and facilitate appropriate support and care• To support clients with activities of daily living, support to attend school, appointments and therapeutic interventions.• To accompany the client getting to outpatient, GP and other appointments relevant to current health or social needs as part of the individual's care plan if agreed to do so.
