We are seeking a dedicated and experienced Locality Social Worker to join our Adult Services team for one of our clients.
The ideal candidate will have a strong background in social work, excellent case management skills, and a passion for supporting adults within the community.
This role involves conducting assessments, developing care plans, and working collaboratively with multidisciplinary teams to ensure the best outcomes for service users.
£33per hour hybrid working available.
Key Responsibilities:
Conduct comprehensive assessments of adults' needs within the community.Develop, implement, and review personalized care plans in collaboration with service users and their families.Regularly monitor and adjust care plans to ensure they meet the evolving needs of service users.Manage a diverse caseload of adults with varying needs, ensuring each individual receives appropriate care and support.Maintain accurate and up-to-date case records, documentation, and reports.Coordinate and facilitate access to services and resources, such as healthcare, housing, and financial assistance.Provide emotional and practical support to adults, promoting their independence and wellbeing.Advocate for the rights and needs of service users within the community and healthcare system.Assist service users in navigating and accessing community resources and services. Education: Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited institution.
Licensure: Reistered with Social Work England.
