On behalf of Walsall Council, Opus People Solutions have an exciting opportunity for a Social Care Facilitator to join their team.
Job Title: Social Care Facilitator Length of assignment: 3 Months with the possibility to be extended Hours per week: 37 hours Hourly rate: £18.26ph PAYE / £24.76ph umbrella Location: Walsall Council - Blakenhall Village Centre, Walsall WS31LZ Remote/Office based: Hybrid working, minimum 2 days office based In this role you will be Main Purpose : You will support vulnerable adults through self-directed support and personalisation, meeting care management and support needs as part of a multi-skilled team.
Key Responsibilities : Assist in creating support plans, promoting choice, independence, and community access.
Contribute to assessments and specialist evaluations in line with self-directed support principles.
Promote citizens' health, wellbeing, and decision-making, ensuring mental capacity considerations.
Collaborate with service users, carers, and multi-disciplinary teams.
Identify needs for simple assistive equipment and refer for more complex needs.
Maintain accurate records and adhere to relevant policies, including the Mental Capacity Act and Health and Safety.
