We are looking for a Community Nurse to work in and around the St Ives and Hunts locality, you will be fully supported to gain new skills and experience needed to deliver high quality nursing care to patients in their own home or residential setting.As a community nurse, we will develop your wound care, palliative care, and long-term conditions, knowledge, and skills; supporting you to become an expert in assessment and care-planning to deliver quality care that is caring and compassionate.If you are looking for a new challenge, or to just progress in your career, come work with CPFT Community Nursing Team.
Main duties of the job We are looking for people who: Have experience of working in the CommunityAre educated to RGN level Are able to demonstrate a commitment to high quality care and service provisionHave the ability to work flexibly and as part of a teamHave the ability to assess and formulate care plansAre able to teach and support student Nurses and Health Care AssistantsHave enhanced communication and inter personal skills You will be responsible for: Keeping the patient at the centre of care, ensuring patient and carer participation in decision makingProviding a high standard of careProviding a high standard of nursing intervention within a patient's own home, with access to Specialist and Advanced Practitioners when requiredWorking as part of a / service and multidisciplinary team Working for our organisation Cambridgeshire & Peterborough NHS Foundation Trust is a health & social care organisation dedicated to providing high quality care with compassion to improve the health & wellbeing of the people we care for, as well as supporting & empowering them to lead a fulfilling life.
