Your role will involve conducting thorough assessments, delivering effective treatments, and offering brief, evidence-based follow-up services to patients referred from community mental health teams, the psych liaison service, or inpatient wards.

Main duties of the job The post holder will support the CRHTT Associate Nurse Consultant, Co-occurring Mental Health Alcohol & Drugs, to support the cohort of patients who are experiencing a mental health crisis which may be complicated by alcohol or substance dependence.
The post holder will also support the team in outreach work for those in mental health crisis which may be complicated by homelessness or unstable housing, ensuring those identified as requiring crisis mental health support are afforded the same opportunities as those where home treatment if the first line intervention available.
Whilst working under the supervision of the Associate Nurse Consultant, this role will hold a degree of autonomy and experience in working with this patient cohort will be key in ensuring success.
Conduct assessments and care planning that demonstrates a high level of expertise in substance and alcohol related complexities for people experiencing mental health crisis.
Develop and expand on relationships with locally commissioned substance misuse services and Local Authorities and VCSE partners to ensure patients receive a holistic approach when managing their mental health needs.

Experience of assessment and case formulation, evidence of skills utilised to manage risk.
