Job summary Were looking for Clinicians that are driven to provide compassionate patient care in challenging, fast-paced, but rewarding environments.An exciting new chapter begins for the PCN, which aims to find likeminded healthcare professionals, to embrace new innovations to help develop and improve the delivery and management of patient centred care to our local population.The Senior Clinical Pharmacist will complement our existing pharmacy team proactively helping patients across the PCN to stay safe, well, and out of hospital whilst supporting efforts to reduce the demands placed on general practice.
Main duties of the job To work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using extensive experience and in-depth knowledge.
The clinical pharmacist will be directly responsible for care and prescribing of defined cohort(s) of patients.To have completed or be near to completing training to become a prescriber to work alongside the general practice team.To deliver and develop pharmacy led medicines optimisation working with general practice and the wider multidisciplinary team, ensuring each practice remains up to date.To lead, promote, support, and facilitate medicines optimisation for specialist area in the wider medicines agenda.
Ensuring best value, reduced harm, and improved outcomes.To lead, deliver, evaluate, and audit service delivery for specialist areas ensuring a high-quality service and to further develop provision.To work to the principles of Medicines Optimisation and relevant up to date national guidelines and evidence base.To support the Prescribing Lead with the medicines agenda within the PCN and primary care practices.

Accurate and timelysummarising of patient records and read-coding patient data.
Complete all requiredpaperwork for legal and administrative purposes in accordance with relevantstandards.
Ensure that all practicepolicies are fully implemented.
Work in accordance withall governance and internal systems relating to (but not limited to) themanagement of clinical data and systems.TRAINING AND DEVELOPMENTTaking responsibility for owndevelopment with relevant evidence-based knowledge and competence in allaspects of the role to meet clinical governance guidelines for ContinuingProfessional Development (CPD) and a Personal Development Plan (PDP).Stay up to date throughattendance at any courses and/or study days necessary to ensure thatprofessional development requirements are met, demonstrating skills and activities toothers who are undertaking similar work.
Subject to a performancereview, including taking responsibility for maintaining a record of ownpersonal and/or professional development.Work closely with otherclinical staff and administrative managers in the setting up and/or improvingof practice systems for monitoring or measuring performance against ClinicalGovernance and Quality Indicator targets.Work to deliver the NHScontract requirements related to the practice (including the terms of theQuality and Outcomes Framework and locally enhanced services)Receive a minimum of onesupervision session per month by a senior clinical pharmacist.Be assigned to a GP clinicalsupervisor for support and development.OTHER DELEGATED DUTIESThis job description isnot intended to be exhaustive - it may be changed after consultation with thepost holder.

Person Specification Experience Essential Minimum of 2 years post qualification experience, including primary care and at least one other field of pharmacy (community or hospital) Good knowledge of NHS medicines optimisation An appreciation of the nature of GPs and general practices Experience of managing / developing a team Understand the aims of current healthcare policy within primary care Has attention to detail, able to work accurately, identifying errors quickly and easily Can effectively manage allocated resources Produce timely and informative reports Excellent understanding of data protection and confidentiality issues Able to think analytically; anticipating obstacles and thinking ahead; using analytical techniques to come up with solutions Excellent verbal and written communication skills with team members, patients, carers, and other healthcare professionals Excellent organisational and time management skills Understanding of budget and health and social care data Committed to own continuing personal development and an ability to support others to develop and progress Desirable In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare Clinical pharmacy experience in running primary care clinics Computer literate with an ability to use the required GP clinical systems Able to analyse and interpret prescribing data Has a planned and organised approach with an ability to prioritise their own workload and meet strict deadlines Politically astute with an ability to sensitively manage complexity and uncertainty Qualifications Essential Educated to Masters degree level in pharmacy, (MPharm), equivalent Registered Pharmacist with the - General Pharmaceutical Council Independent prescriber or working towards/intent of gaining independent prescribing qualification Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience Educated to Masters degree level in pharmacy, (MPharm), equivalent Personal Qualities Essential Works effectively independently and as a member of a team Flexible approach to meet service needs and ensure a stakeholder focused response Self-motivated and proactive Continued commitment to improve skills and ability in new areas of work Able to undertake the demands of the post with reasonable adjustments if required Able to work across several sites and travel to meet with colleagues Confident in leading a team Other Requirements Essential Commit to a DBS check Flexibility to work outside of core office hours
