Main duties of the jobIn accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions, and dealing with queries, paperwork, and correspondence in a timely fashion. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers within the organisation. About usThe West Park Surgery is an Outstanding CQC rating at the Long Eaton Health Centre site and at the branch site of Park View Medical Centre. We are a secure, successful, and forward-looking practice. The GP Partners hold key leadership positions in the PCN and the ICS and can provide a wealth of opportunity and experience to other members of the team. The practice is heavily involved in supporting and developing PCN services across the whole of Erewash.
We are part of Erewash Health Partnership and have access to a wide range of networked services including home visiting, care home support team, pharmacists, social prescribers, and first contact physios. We offer all core services as well as minor surgery and contraceptive services.
Education remains an important part of the ethos of the practice, and we train registrars, medical students, non-medical prescribers as well as supporting apprentices from the local community. This culture of continuous improvement and development is core to who we are. Regular clinical meetings across both sites, network learning events, and an allocated mentor mean that we create a safe and supportive environment.
Job responsibilitiesAssessing the health care needs of patients with undifferentiated and undiagnosed problemsScreening patients for disease risk factors and early signs of illnessDeveloping care plans for health in consultation with patients and in line with current practice disease management protocolsProviding counselling and health educationAdmitting or discharging patients to and from the caseload and referring to other care providers as appropriateRecording clear and contemporaneous consultation notes to agreed standardsCompiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriateIn general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary careAwareness of and compliance with all relevant practice policies / guidelines, e.g. prescribing, confidentiality, data protection, health and safetyA commitment to life-long learning and audit to ensure evidence-based best practiceContributing to evaluation / audit and clinical standard setting within the organisationContributing to the development of computer-based patient recordsContributing to the summarising of patient records and read-coding patient dataAttending training and events organised by the practice, PCN or other agencies, where appropriatePerson SpecificationExperienceTelephone triageHome visitingExperience of working as duty doctorFully qualified GPMedical educationProven track record of delivering QOFHigher postgraduate qualificationsQualificationsKnowledge of GP contracts/enhanced services/QOFKnowledge and completion of CPD to remain up to date with GP appraisal/revalidationAbility to listen and empathiseAbility to work/lead as part of a multidisciplinary teamThorough understanding of clinical governanceKeyboard skills and computer literacyAble to work under pressureSelf-motivated and positiveRecognises own limits and acts appropriatelyExperience of multidisciplinary workingGMC full registrationCurrently on a performers listKnowledge of local agendas in respect to primary careKnowledge and understanding of PCNSpecialist interestsAbility to lead in clinical areas/QOF/ES domains within practiceEnhanced skills e.g. joint injections, minor surgery, education, family health etcExperience of SystmOneDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
