The RoleAs a Referral and Allocations Partner, you will receive direct referrals and enquiries, providing a single point of contact for the Social Reablement and Crisis Support Service, working alongside partner agencies to ensure people accessing the service receive the correct support in a timely manner.
You will work in conjunction with the Home from Hospital Service ensuring the service is as easy to access as possible.

ResponsibilitiesThe appointed Referral and Allocations Partner will:
Be person centred at all times and work with people to identify what is important to them and ensure the most appropriate intervention provides an effective route to support.Deliver a 'no wrong door' approach, providing a supported referral into other services where more appropriate.Work directly with the Social Reablement team, allocating cases effectively to the most appropriate intervention.Work collaboratively with statutory and non-statutory services including but not limited to Hospitals, CMHTs, Substance Misuse Teams, Healthcare professionals, social prescribing initiatives, early help and preventative approaches of partner agencies, providing a coordinated and holistic service involving advocacy and crisis response.YOU has a strong value base, striving to be person-centred, innovative, trustworthy, can-do and excellent at all times, so it is important that you are as passionate about these principles as we are.
