In most cases, an Executive Directorwill be accountable for each project and the post holder will report to thisExecutive Director (SRO) and Head of Strategy & Planning, the SeniorResponsible Owner (SRO) for the project and PCN Executive as appropriate, fordelivery of the relevant project.Job responsibilitiesTo lead in the development,planning, implementation, management, monitoring and evaluation of specificsmall to medium sized projects or change initiatives, as part of the wider programmeof change for THE PCN working closely with key internal and externalstakeholders; supported where appropriate.To lead and implement a strategic plan that includes arobust digital infrastructure, including supporting PCN and practice staff touse new digital systems.Coordinate training and development of PCN staff.Achievement and reporting of primary care improvement targets and initiatives, including IIF/QoF.To provide oversight and accountability to the delivery of PCN contracts and specificationsTo support the implementation of objectives and priorities for network delivery in line with operational capacity, financial parameters and contract requirementsPromote the engagement of all PCN practices and act as an ambassador for the network.To proactively build relationships with system partners to help influence proactive and new models of careTo use business intelligence tools to aid data-driven decisionsIdentify and record risks and issues and develop contingency plans with Network LeadsEnsure all relevant national strategies, policies, targets, and frameworks are fully implementedTo drive issues through to resolution, acting as a point of expertiseTo implement network-based projectsTo support the collation of financial informationTo monitor and check the accuracy of the submissions for the DES and contractual requirementsCompiling and presenting a range of performance and financial reports for the PCN and regional/national authoritiesTo ensure that the PCN's physical and financial resources are managed and maintained for maximum effectiveness.To ensure the PCN maximises its business intelligence toolsDevelops a continuous improvement approach to digital accessibility for our patients and workforceCreate data packs to support decision-making that includes but is not limited to demand, capacity, utilisation data, staff and patient experienceTo foster a culture of collaboration and promote learning to drive continuous improvementTo work as a flexible member of the PCN, providing support to other team members when necessaryDevelop and deliver a communication plan using a wide variety of channelsTo take an active role in the development and embedding of the PCN's culture, values and reputationTo represent the network at external meetingsSupport recruitment, induction and training of new PCN staffSupport recruitment which is aligned to a workforce planTo facilitate progression for the PCN staff to promote retentionOther:Any responsibility identified duringthe course of the job.

Person Specification Experience Essential KNOWLEDGE: Proven ability and experience of leading or supporting change programmes Experience of facilitating learning, public speaking, conference presentations, in Trust wide meetings.
Experience of collation, analysis and dissemination of information or data Experience using software packages to support change (such as Word, Excel, PowerPoint, MS Project, and Visio).
Experience using EMIS web to produce reports and manage QI work such as QOF including building reports and templates EXPERIENCE: Significant experience of working in NHS, specifically primary care Experience of managing transformation projects Experience of managing digital projects SKILLS: Emis search and reporting Able to build and secure credibility with colleagues and stakeholders.
