Job summary A fantastic opportunity has arisen for a motivated Physiotherapist or Occupational Therapist to join our highly skilled and experienced Early Intervention Team at Cambridge University Hospitals.We are looking for an innovative and motivated team player, working collaboratively with other members of the multidisciplinary team, including our Community partners, to provide a holistic, patient-centred admission avoidance service.You will support speedy discharges for patients with long term conditions and complex health needs.
You will provide a specialised therapy assessment in order to prevent the unnecessary admissions of patients into acute care or to begin their acute rehabilitation.You will be responsible for providing a high standard of assessment, advice, treatment and evaluation of patients to formulate treatment plans for patients.
This may include patients with diverse presentations and complex physical and psychological conditions.The role will allow development of specialist knowledge of ED and Frailty specific assessments, with an opportunity to evaluate practice and audit outcomes.
Development opportunities will include continuing professional development, links with Community Services, contribution to operational group meetings and striving to meet our national and local guidelines.You will be expected to contribute to seven-day working between the hours of 8am and 8pm, based on a rota system.
Main duties of the job Responsible for the provision of high quality, patient focused care.Responsible for assessing, developing and implementing specialist therapy provision in the Emergency Department, Short Stay Wards and G2 Frailty Unit.Responsible for providing specialist advice and support to, and liaising with, other staff to support admission avoidance and early supported discharge where appropriate.

Person Specification Qualifications Essential Registered Physiotherapist / Occupational Therapist Evidence of continuous professional development Post registration qualification in speciality or willingness to complete Desirable Degree Research Module Experience Essential Demonstrate expert therapy skills from a broad range of clinical environments -Experience in Orthopaedics, Elderly Medicine, Neurology and Musculoskeletal rotations as a Band 5/6 Accept personal accountability for own practice Experience of formal / informal teaching Involvement in service development Experience supporting therapy support staff and students Desirable Experience working in the Emergency Department or Short Stay Wards Knowledge Essential Understanding of evidence based practice Understanding of Clinical Governance Good understanding of dealing with patients with complex medical conditions such as fractures, stroke, functional neurology disorders and mental health issues Knowledge of community services, complex discharge planning and important legislation Understanding and knowledge of admission avoidance Understanding and knowledge of Frailty Skills Essential Good verbal and written communication skills Variety of clinical skills working with and supporting complex patients Capable of working as an integral member of a small team Flexible and adaptable to changing situations due to both workload and colleagues needs Ability to undertake teaching of staff and patients Ability to work autonomously and manage own workload; effective time management Effective interpersonal skills Commitment to efficient and effective multidisciplinary working Desirable Assertiveness / perceptive counselling skills Documentation, audit and presentation skills Leadership skills Additional Requirements Essential The ability to understand and behave at all times, towards patients, visitors and colleagues according to the Trust values of Safe, Kind, Excellent.
