Occupational Therapist

We are looking for an Occupational Therapist to provide assessments and interventions within a multidisciplinary team with an aim to support residents within their home environment in Maidenhead.
To work specifically within our Integrated Discharge and Flow Pathways in the post Discharge space.
? To work alongside Carers, Physiotherapists, and other members of the team to maximise potential and establish long-term needs in the first few weeks post discharge.
? To advocate for the resident and ensure needs and wishes are represented working closely with family and significant others.
? To work with people to maximise potential and endeavour to keep residents in their own homes.
? To work with residents in Care homes where placement is interim, with the aim of supporting and enabling residents to return home where possible.
? To provide day to day ongoing professional support to residents as required and where possible, promote social change.
? Monitor and review Occupational Therapy outcomes in line with policy and procedures.
? Assist in multi-disciplinary problem solving to enhance wellbeing using relevant professional skills.
? To work in an inter-disciplinary way with the rest of the team, understanding their roles and developing close integrated working ways.
Joint visits with other team members are the norm.
? To plan workload alongside the whole Discharge Therapy Team and to be flexible in this approach.
Pay Rate- £34-£36 per hour
