Proficient in English literacy and numeracy, with strong interpersonal and communication skills.
Skilled in using or willing to learn information technology systems for data management and communication.
Capable of assessing and providing clinical reasoning for recommendations, including assistive technologies, equipment, and support plans.
In-depth knowledge of legislative frameworks such as the Care Act, Mental Capacity Act, and Safeguarding, and experience working with statutory and non-statutory agencies.
Autonomous worker, able to plan and organize caseloads while prioritizing urgent needs and risks to service users and carers.

What the Job Involves Conducting assessments for older people, adults with disabilities, and disabled children in accordance with the Care Act 2014 principles.
Providing advice and making recommendations to enhance wellbeing, maximize independence, and support carers.
Ensuring high-quality standards in assessments and service delivery.
Implementing and managing Safeguarding Adults policy and Health and Safety policies, keeping abreast of developments in professional practice and legislation.
Managing, monitoring, and reviewing care plans in collaboration with users, carers, and other agencies.
Developing and managing data systems related to team functions, entering case information, and collaborating with Administrative Officers.
Encouraging customer participation in service delivery, initiating research, and implementing policy changes.
Undertaking complex assessments and care planning, including functional assessments and carer assessments.
Advising on care packages, referring to other services, and recommending specialist equipment or major adaptations for properties.
Working with vulnerable adults, housing, health and adult social care staff, partner organizations, and other stakeholders.
