Office bases are in Ellesmere Port, Chester and Winsford and the expectation is that staff work in the office - times a week and at home or out on visits the remainder of the week.The Complex Care and Review team is a growing, passionate, and creative team consisting of a Team Manager, Practice Manager, business support officer, x Social Workers, x Social Care Assessors, x Occupational Therapist and x OTA.
This is an experienced team of highly motivated, hardworking, and skilled professionals.
We are looking for candidates who demonstrate these skills and have a passion to drive forward positive outcomes for individuals regardless of their level of need or stage of life they are in.
Candidates will be required to work collaboratively and creative with colleagues, care providers, and housing developers and to have a sound knowledge of current legislation, policy and procedures within adult services.The OT will complete strength based assessments (Mental Capacity Assessments when required and necessary to do so) and offer advice in relation to levels of support required to meet care needs, assess for, recommend and provide equipment and adaptations to support independence and complete moving and handling assessments.
The role involves the exciting opportunity to be involved in bespoke projects, advising on environmental design to meet individual needs within new services and accommodation set up by the team to fill gaps in resources identified for our client group.We provide high-quality, person-centred social care for adults, but for us it's more than that; we support people to change and enhance their lives every day through the work undertaken by our talented and dedicated teams.

You will have: Degree or diploma in OT (Essential).Up to date registration with HCPC (Essential).Be required to undertake an enhanced DBS plus Adults and Childrens Barred List (Essential).Experience of working in a Social Care and/or Health community settingAwareness of importance of multi-disciplinary working and an integrated approach.Experience of liaising and working effectively with clients, (adults and children) their families/carers, other professionals, and agencies to achieve the best outcomes for people.Demonstrable working knowledge of Health and Social Care Policy, housing and legislation affecting general OT and moving and handling practice.Ability to assess and manage complex clinical risk.Ability to cope positively with change and an ability to promote positive changes to make a difference.Awareness of importance of multi-disciplinary working and an integrated approach.You are confident, energetic, and enthusiastic as well as sensitive and resilient.Self-motivating individual with the passion to make a difference in adult social care.Resilient personal stress management.
