Job summary The mental health practitioner will sit within a well established Homebased treatment team consisting of nurses, Occupational therapists, social workers, Support workers, psychological therapists, medics and ACPS.They will work within the urgent care services to provide least restrictive care to individuals in crisis to support community recovery providing 24 hour access to care to those in crisisThe post holder will have access to ongoing training to provide psychosocial interventions to those in crisis including those with complex emotional and relational needs, SMI and sever depression and anxiety.The post holder will work within the MDT to take positive therapeutic risk to work with service users to manage risks.
Main duties of the job The post holder will work as part of an integrated multi-disciplinary community service, with responsibility for assessing health and social care needs, providing a gatekeeping role within the Bolton directorate.
They will provide clinical interventions focusing on relapse prevention, early discharge and improved quality of life for service users who are in crisis.
The post holder will be expected to offer highly developed risk assessments and management plans with an emphasis on Recovery.
The post holder will be required to act as shift co-ordinator, taking responsibility for the triage and assessment of referral, the allocation of the team's work, including assessments in A & E, community and on the wards and the provision of home based treatment.
Offer assessment, support and care for individuals brought to Bolton's identified Place of Safety facilities in relation to Section 136 of the Mental Health Act 1983.
The post holder will assist the management and leadership team in the day-to-day operational running of the team, organising, leading, teaching, supervising and supporting junior colleagues.
