Job summary We have a fantastic opportunity to come and work for the Major Trauma Rehabilitaion Coordinator Team (MTRC).
As the Peninsula Trauma Centre, University Hospitals Plymouth has an established team of MTRCs with the trust due to open the Major Trauma ward in December 2024, it is an exciting time to join us.
Working as a Rehabilitation coordinator, you will be informed of all Major Trauma patient admissions for all specialties, coordinating the ongoing care including discharge planning, transfer and rehabilitation needs of patients.
You will act as a single point of contact for liaison with Major Trauma Centres and Trauma units to arrange repatriations, gaining thorough handover of patient status in particular completing the Rehabilitation Prescription.
You will need to be confident and proactive in your communication skills and caseload management.
You will actively participate in relevant Trauma Network and Trust meetings, reviews, carry out local and national audits and support the development and review of procedures, guidelines and protocols.
The role as well as being busy and varied will also enable you to consolidate your clinical knowledge and develop skills as a Band 7 in project management, audit and service development.
***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.
*** Main duties of the job The MTC Rehabilitation Co-ordinator will be responsible for assessing, co-ordinating and overseeing the rehabilitation for all major trauma patients during their acute phase of care, through to discharge for rehabilitation or repatriation within the Network and beyond.
They will work with the MTCC's and ward MDT to holistically assess the rehabilitation needs and goals for all MTC patients.The MTC Rehabilitation Co-ordinator will co-ordinate the weekly rehabilitation multidisciplinary team meeting to discuss all current in patients.
Provide clinical coordination to ensure that all rehabilitation services work collaboratively to improve the rehabilitation provided, with a focus on the patient achieving the best possible outcome.The MTC Rehabilitation Coordinator will have regular communication with multidisciplinary teams within the TUs, Rehabilitation units and community teams and will handover patient's rehabilitation goals as necessary and be responsible for on-going development of and completion of the Rehabilitation Prescription.There will be a responsibility to provide support to other clinical staff within the hospital and to provide teaching sessions as needed in relation to rehabilitation after major trauma.The MTC Rehabilitation Co-ordinator will participate in completion of nationally required audits and reviews results from PROMS and will review outlier patients.

Job description Job responsibilities To be the rehabilitation specialist within the Peninsula Major Trauma Centre Coordinator Team.
To provide holistic assessment within the first 72 hours of the inpatient stay, including commencement of a rehabilitation prescription.
To ensure that a complete rehabilitation prescription is delivered for every patient.
To lead on the development of links to rehabilitation services within Trauma Units, Rehabilitation Units and Community teams across the Peninsula Major Trauma Network.
To provide point of contact and co-ordination for rehabilitation matters for all major trauma patients during their hospital admission and to work with existing teams to facilitate transfer to appropriate rehabilitation services in the Peninsula Trauma Network.
To be a point of contact for the patient, GP and other health professionals regarding rehabilitation matters after discharge from the MTC.
To provide effective case management for trauma patients wherever they may be in the MTC, ensuring appropriate medical and therapies review(s) and thereby reducing delays.
Person Specification Knowledge & Experience Essential Demonstrable post graduate experience in a rehabilitation setting (inpatient rehabilitation unit or community rehabilitation team) Demonstrable experience in rehabilitation Desirable Demonstrable clinical experience of specialist inpatient rehabilitation Specialist proven knowledge of acute rehabilitation of multiple trauma / spinal cord injuries/traumatic brain injury Aptitude & Abilities Essential Excellent communication skills, both written and verbal, with ability to demonstrate fluency, clarity and effectiveness at all levels Effective influencing or negotiating skills: both in general and with senior health care professionals Desirable Ability to motivate and inspire others Proven evidence of strong analytic and judgement skills Qualifications Essential NMC or HCPC Registration To hold a master's degree or equivalent proven knowledge and/or equivalent demonstrable experience of major trauma rehabilitation Desirable Management training
