Job Summary:
The post-holder will manage apersonal list and deal with a wide range of health needs in a primary caresetting, ensuring the highest standards of care for all registered andtemporary patients.
Job Responsibilities:
The post-holder will undertake a variety ofduties including surgery consultations, telephone consultations andqueries, visiting patients at home, checking and signing repeatprescriptions and dealing with queries, paperwork and correspondence in atimely fashionAssessing the health care needs of patientswith undifferentiated and undiagnosed problemsScreening patients for disease risk factorsand early signs of illnessDeveloping care plans for health inconsultation with patients and in line with current evidence based on practicedisease management protocolsProviding counselling and health educationReferring patients to specialists and to other care providers as appropriateRecording clear and contemporaneous IT basedconsultation notes to agreed standardsCollecting data for audit purposes and to meetpractice targets (e.g QoF)Compiling and issuing computer-generated acuteand repeat prescriptions (avoiding hand-written prescriptions whenever possible)Prescribing in accordance with the practiceprescribing formulary (or generically) whenever this is clinically appropriateIn general the post-holder will be expected toundertake all the normal duties and responsibilities associated with a GP working within primary careParticipate in agreed practicedevelopment activities.Attend training, practicemeetings and events as appropriate.Participate in supervisinglearners within the practice (GP Trainees and F2)Person SpecificationQualificationsRegistered with the GMCDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
