The role will be varied,challenging and rewardingas you support and develop the servicethrough implementation, deliveryand ongoing innovation and service development.Key responsibilities will be: Ensure the NHS Health Checks service is of highprofile and there is strong awareness in relation to the criteria of entry inthe service.
Assist the Head of Service to develop the integratedhealth offer within Dudley and raise the profile of ABL Health.
Identify and engage with individuals from targetgroups and communities to facilitate communication regarding promoting theirhealth and wellbeing, and the Lifestyle service.
Attend health events across the borough, undertakehealth checks and promote the wider Dudley service Develop and maintain relationships with individualswho are experiencing the greatest inequalities in health and provideinformation to individuals about the relationship between behaviours andhealth.
Make contacts, develop and maintain professional linkswithin communities including promoting the Health Checks Service and your roleto contact groups where there may be barriers to understanding.
Liaise with key partner organisations such as,workplaces, community organisations, community centres and faith groups toincrease referrals into Your Health Dudley services.
To support all the Your Health Dudley services toensure quality and governance is robust Promote the integrated Lifestyle Service and allcomponents within the local community To champion andadhere to processes and procedures to identify and strive for continuing improvedquality throughout the patients pathway, together with robust data management.
Deliver against an agreed set of KPIs.
To meet internal and external performance managementrequirements Be responsible for ensuring rigorous and regular datainputting to ensure accurate records are maintained The post holder is expected to work within therequirements of the 6 Cs Care, Compassion, Competence, Communication,Courage and Commitment.

Trainexisting and new staff to delivery Health Checks Annualcompetency and observation checks for all staff delivering Health checksAttending local health forums/groups/meetings where required torepresent ABL, promote the serviceand maintain local partnerships Support the Head of service to develop robust SOPsSupport the Head of Service to complete 10% audits of all work carriedout Engage with GP surgeries and Primarycare networks to develop case finding and population health management projectsto drive referrals into all elements of the service Develop partnerships with secondarycare and inpatient services to increase smoking cessation referralsSkills and Competencies RequiredEmbed referral pathways to ensure efficiency and quality referrals intothe service Training expertise Experience of building and maintaining community partnerships Build strong relationships; Fostertrust and co-operation among colleagues, stakeholders, community leaders,commissioners andsub-contractors Quality driven; you naturally seek high standards and actively seek to improvethem.
