Job summary We are Trafford Local Care Organisation - an exciting, new public sector organisation that is bringing together NHS community health services and social care services in Trafford.

Vacancies have arisen for Community Specialist Practitioners (CSP) in North Trafford .
You will be required to work as part of our developing Neighbourhood team to lead and co-ordinate nursing teams to provide complex nursing care to adults in their own home and in a variety of settings within the community.
You will work closely with the MDT (Social Workers, GP's and Allied Health Professionals) to ensure that patient care is effectively co-ordinated, demonstrating a positive impact on our patients.
You will enjoy the chance to develop new and existing clinical skills, as well as a range of training opportunities for personal development.
We will be looking for proven leadership skills and the ability to work within and lead and develop more junior members of staff, as well as work autonomously.

Main duties of the job You will hold the relevant CSP Qualification and have the required community nursing experience as set out in the job description.
