Job summary An exciting opportunity has arisen for a B4 Community Senior Rehabilitation Assistant to work with the Intermediate Care Team in South Manchester.
Our Pathway two rehabilitation beds are based at Buccleuch Lodge Intermediate Care Unit in West Didsbury.
The team also supports patients in their own environment on our 'Home Pathway'.
The Home Pathway Team are based at Trafford House in Wythenshawe.The post is part of our Manchester Community Response Service which also includes the Crisis Response team, Home Pathway, Reablement, Community IV team, Pharmacy and Social care.This is an exciting time to be part of our integrated multi-disciplinary and multi-agency service providing rehabilitation.Buccleuch Lodge provides inpatient care to 14 patients, providing rehabilitation with the aim of returning patients home with support.
We have a unique Community setting with dedicated nursing, therapy and social care staff.We currently have a full time vacancy for a B4 Therapy support worker who is compassionate, forward thinking and a team player that wants to progress within the environment of community rehabilitation.The developments in the service include working towards a 7 day service with extended hours.Previous applicants need not apply.
Main duties of the job The post holder will work as an integral part of the Intermediate Care Therapy team (and the wider MCR service) to provide a high quality patient centred approach to adults in the community in South Manchester.The teams integrated service delivery model aims to avoid admissions, reduce length of stay and improve the patients' experiences through access to the right intervention at the right time, delivered by the appropriate health or social care practitioner.The aim of the post is to rehabilitate and improve the function of patients by providing interventions in their own home / Buccleuch Lodge, either following discharge from an acute hospital setting or following illness or injury at home.After completing appropriate competencies, you will be expected to manage your own caseload of patients, both in our bed based unit and patient's own home.You would be part of a wider multi disciplinary health & social care team who work towards regaining or maintaining independence and quality of life.
The post holder will have an allocated caseload of patients under the supervision of a registered professional.The role will also involve team/office based duties to ensure effective service delivery.The team is committed to continual professional development with regular supervision and appraisals.
This post is supported by B5, B6 and B7 Physiotherapists and Occupational Therapists.The role will also involve close working with students and supporting the B3 support worker.

Person Specification Education/Qualifications Essential Multi-professional NVQ Level 3, or equivalent.
Numeracy and Literacy skills (to GCSE level or equivalent) Desirable Foundation degree in Health and Social Care Experience Essential Experience in working at the level of a Therapy/Rehabilitation Assistant carrying out therapeutic interventions.
Experience of working as part of a multi-agency, multi-professional team.
Evidence of working unsupervised.
Desirable Experience of working in falls/general rehabilitation in acute or community.
Experience of training other staff.
Experience as a care/voluntary work.
