Job summary Community Psychiatric Nurse - Newport Older Adult CMHT Join our dynamic and supportive Older Adult Community Mental Health Team (CMHT) in Newport, where you will play a pivotal role in delivering high-quality mental health care to older adults with complex mental health needs.
As part of our multidisciplinary team, you will provide holistic, person-centered care, including comprehensive assessments, care planning, risk management, and therapeutic interventions.
In this role, you will work collaboratively with patients, families, carers, and other professionals to ensure continuity of care and positive outcomes.

Main duties of the job The Community Psychiatric Nurse will work within their code of practice to take responsibility for their allocated caseload to ensure assessments of health and social needs.
This will include formulation, planning and evaluation of care which include regular reviews and update of risk assessments documentation and care planning.
They will be a care co-coordinator and advocate for the service user at times and promote independence through the recovery process.The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
