Clinical supervision and a supported induction period are provided.For further details / queries, please contact: Jenny Jones 0300 123 5007 Option 1 Main duties of the job The Community Neuro Rehabilitation Team is based at the Adelaide Health Centre in Southampton.The team provides multi professional neurological rehabilitation in the community.We are a busy service with a diverse caseload working across the city.
We provide specialist assessment, treatment and advice, both in clinic and community settings, working collaboratively with the patient, primary and tertiary services.Purpose of the Position:*To assist the Community Epilepsy Specialist Nurse in the provision of the specialist nursing service for all adult patients with Epilepsy across the Southampton City CCG boundary.
*To manage a caseload of clients requiring specialist epilepsy nursing, within the community.
*Undertake assessment and treatment of clients within both clinic and community setting, who have complex / and or chronic presentation requiring specialist skills; and determine profession specific and multidisciplinary rehabilitation interventions.
*To provide advice, education and support to patients, their carers' and healthcare professionals and assist in coordinating treatment and care.
*To work across professional, departmental and Trust boundaries to support the delivery of a patient-centred seamless service.
*To work in the Community Neurological Rehabilitation Team (CNRT) in close collaboration with the Epilepsy Specialist Nurse, delivering and evaluating care pathways for clients with long term neurological conditions.

Person Specification Qualifications Essential Registered General Nurse (RN, RNLD) (or equivalent other NMC).
Degree or Diploma in Nursing.
Portfolio evidence of post registration learning.
Desirable Evidence of post registration learning in epilepsy.
Experience Essential Experience of managing patients with epilepsy.
