Job summary An exciting opportunity has arisen for a Band 5 Registered Nurse to join our forward-thinking West Suffolk Community Cardiac Rehabilitation Team, to contribute in the delivery of a high standard of holistic health care to patients within clinics, leisure centres and their own homes, by providing Cardiac Rehabilitation as part of the multi-disciplinary team.
This role can be offered as a developmental role and training can be provided for the right candidate who is keen to pursue an exciting and rewarding career in this evolving speciality.
Cardiac rehabilitation service provides person-centred care that enables individuals to increase independence, improve health outcomes and reduce unnecessary admissions to hospital or long-term care.
This is achieved through timely interventions, rehabilitation, and management of long-term conditions.
We are looking for a passionate and motivated Nurse who is keen to develop evidence-based knowledge to provide specialist clinical care in both cardiovascular disease and other cardiac conditions.
The successful candidate will need assessment skills to initiate clinical treatments, provide education and appropriately refer onward to our health and social care partners along with some voluntary sector services.

Job description Job responsibilities MAIN RESPONSIBILITIESClinicalThe post holder will: Triage, assess, plan, implement and evaluate all aspects of patient care, and develop care plans which mayneed to be delivered from a range of options.
Ensure informed consent is obtained and documented prior to initiating interventions.
Following a holistic assessment, devise an individualised, evidence-based plan of care for each patient,modifying it as required.
Assess patients holistic needs, communicating complex and sensitive information to patients and carers.
To identify and assess patients risk factors in relation to cardiovascular disease using recognised tools To formulate an individual plan of cardiac rehabilitation in collaboration with each client taking into accountindividuals ability and understanding in order to make lifestyle changes Use tactful and persuasive skills (such as motivational interviewing) to gain the patients co-operation in theircare management plan.
To assess psychological function using recognised assessment scores HAD score or PHQ9 / GAD7 To assist senior staff with provision of education classes to support and advise patients with their rehabilitationfollowing cardiac events To refer patients appropriately to dietician or smoking cessation services To participate in CR group exercise sessions mirroring the patient.
Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sectorto ensure identified needs are met and care co-ordinated.
Maintain skills with regards to latest technology Participate in multi-disciplinary/multi-agency meetings as appropriate, Gold Standard Framework.
Ensure own workload is planned and prioritised according to patient needs.
Ensure patient held records are completed for each contact, and that SystmOne inputting is completed on adaily basis.
Could be exposed to bodily fluids, infected material, blood products, therefore must utilise universal precautionsand adhere to infection control policies.ProfessionalThe post holder will: Actively contribute to multidisciplinary team meetings and other meetings.
Support all team members within the integrated team, respecting them, their roles and contributions.
Be responsible for safe use and maintenance of equipment and supplies and be aware of budgetary constraintsand work within these.
Be accountable for own professional actions as determined by the Nursing and Midwifery Council code.
Provide advice / training to less experienced staff and supervise / support staff and students, contributing to theinduction process for new staff/students.
Comment on draft policies.
Maintain own continuous professional development and attend training as required to develop job role Be accountable for work you have delegated to others within the team, ensuring staff are competent to undertakedelegated work.OrganisationalThe post holder will: Ensure adherence to Trust policies, and procedures.
Undertake mandatory training as required.
Undertake Personal Development Plans for junior staff members.

Ensure Trusts behaviour framework is utilised when managing and supervising staff, and that behaviours areembraced and embedded within the team.
Take part in clinical supervision as per Trust Policy.
Demonstrate own duties to students, new starters and/or less experienced staff Provide mentorship for students undertaking pre/post registration course.
Provide Preceptorship to new staff as appropriate Contribute to clinical audit as required.
Complete risk assessments and incident forms as required.Research & Development Will be required to undertake surveys or audits, as necessary to own work; may occasionally participate in R&D,clinical trials or equipment testing.
