Role Responsibilities: Optimize medication use for patients across GP practices in York Provide expert advice on medicine management and contribute to medication reviews Work alongside multidisciplinary teams to deliver high-quality care Participate in continuous professional development and clinical audits Engage with patients to improve their medication adherence and outcomes Requirements: GPhC registered pharmacist 2+ years of experience in clinical pharmacy, ideally in primary care Excellent communication and teamwork skills A passion for improving patient care and outcomes If you would like more information on this role, give us a call on 020 4585 0659.
