Duties and Responsibilities: Conduct medication reviews to ensure safe, effective, and appropriate prescribing.
Provide expert advice on the use of medicines to patients and healthcare professionals.
Support chronic disease management through medication optimization.
Contribute to clinical audits and quality improvement initiatives.
Collaborate with multidisciplinary teams to enhance patient care.
Qualifications: Registered as a Pharmacist with the General Pharmaceutical Council (GPhC).
Proven experience in a clinical setting, preferably within primary care.
Strong communication and decision-making skills.
Ability to work autonomously and as part of a team.
