Job summary The Clinical Pharmacist iscrucial for providing comprehensive pharmaceutical care, enhancing patientoutcomes through effective medication management.
This role involves workingunder the supervision of the Senior Clinical Pharmacist to review, dispense,and manage medications, ensuring their safe and effective use.
This role will be based within our Organisational Pharmacy Team at Stirling road Medical Centre, but will be working for Coleridge Medical Centre which is also part of the Fuller and Forbes Healthcare Group.Key responsibilitiesinclude conducting medication reviews to optimise therapy, managing druginteractions, providing patient counselling, and supporting the healthcare teamin prescribing decisions.
The Clinical Pharmacist will also participate inhealth promotion initiatives and contribute to the continuous improvement ofpharmaceutical care practices.This position requires adeep understanding of pharmacology, excellent communication skills, and adetail-oriented approach to patient care.

Our service is undertaken by our vast multidisciplinary team of GPs, Advanced Clinical Practitioners, Pharmacists, Paramedics and Nursing teams, as well as our PCN staff.
Job description Job responsibilities Medication Management:Medication Reviews: Regularly review patient medications to optimise therapeutic outcomes and minimize risks of adverse effects.
Adjust prescriptions as necessary in consultation with the Senior Clinical Pharmacist and other healthcare providers.Prescription: Ensure prescription requests, acute and routine, are prescribed,.Drug Interactions and Contraindications: Vigilantly monitor for potential drug interactions and contraindications to prevent medication errors and ensure patient safety.Discharge Resolutions: resolve medication issues and requests from hospital correspondence.Pharmacy Tasks: complete the wide range of medicines-management pharmacy tasks on a daily basis.Patient Care and Counselling:Patient Education: Provide comprehensive counselling to patients on medication use, side effects, interactions, and storage, enhancing patient knowledge and compliance.Clinical Decision Support: Assist healthcare providers in making informed clinical decisions regarding pharmacotherapy, utilising up-to-date clinical data and evidence-based guidelines.Chronic Disease Management: Participate in the management of chronic diseases by providing expertise in pharmacotherapy, contributing to patient care plans, and monitoring patient progress.Collaboration and Communication:Multidisciplinary Team Collaboration: Work closely with a multidisciplinary team including doctors, nurses, and other healthcare professionals to ensure a coordinated approach to patient care.Healthcare Team Support: Provide drug information and pharmacological advice to healthcare team members, supporting the safe and effective delivery of treatments.Patient Interaction: Engage with patients to gather information on their health status, medication history, and treatment responses to inform care decisions.Quality Improvement and Compliance:Quality Assurance: Participate in quality assurance programs to maintain high standards in pharmacy services and patient care.Regulatory Compliance: Ensure all pharmacy activities and procedures comply with relevant laws, regulations, and standards, including GDPR, clinical governance, and professional pharmacy practice.Audit Participation: Assist in conducting audits of pharmaceutical services to identify areas for improvement and ensure compliance with best practice standards.Professional Development:Continuous Education: Engage in ongoing education and professional development to stay current with advancements in pharmacology and pharmacy practice.Research Involvement: Contribute to research projects and studies within the practice to advance knowledge in pharmacotherapy and improve clinical outcomes.Training and Mentoring: Support the training and development of pharmacy staff and students, providing mentoring and guidance to enhance their skills and knowledge.Miscellaneous:The Practice recognises that the role above is wide and varied is evolving in nature.

Person Specification Standards Essential Additional Competencies: Commitment to ethical practice, maintaining high standards of integrity and confidentiality in handling patient information.
Flexibility and adaptability in a dynamic healthcare environment, able to respond effectively to changes in practice or patient needs Qualifications Essential Educational Background: MPharm degree required or equivalent.
Registration as a pharmacist with the General Pharmaceutical Council (GPhC) or equivalent professional body is mandatory.
Experience: Proven experience as a Clinical Pharmacist involved in direct patient care and medication management.
Experience working within a multidisciplinary healthcare team is highly preferred.
Demonstrated ability to manage medication therapy including reviews, dispensing, and monitoring of drug interactions.
Technical Skills: Strong clinical pharmacology knowledge, with the ability to apply this practically in patient care settings.
Proficiency in using Electronic Health Records (EHR) and other pharmaceutical software, essential for effective medication management and patient documentation.
Regulatory Knowledge: Thorough understanding of pharmacy regulatory standards and compliance requirements, including GDPR, medication safety protocols, and clinical governance.
Communication Skills: Excellent communication skills, capable of effectively discussing treatment plans and medication information with patients and healthcare professionals.
Ability to convey complex medication-related information clearly and succinctly to ensure patient understanding and compliance.
Desirable Patient Care Focus: Strong focus on patient-centred care, with the ability to assess patient needs, preferences, and individual responses to therapies.
