Job summary Clinical Nurse Specialist - Colorectal Clinical Nurse Specialist Band 6 We are seeking a compassionate, dynamic and motivated nurse to join our well established and supportive colorectal nursing team.
You will be an integral part of the team providing coordination and organisation of care for patients along the cancer pathway.
You will need good communication and interpersonal skills and a good knowledge of the care needs for colorectal cancer patients and their carers.
This is an opportunity for nurses who have experience within colorectal cancer setting and want to further enhance their skills and knowledge within this speciality The post holder is responsible for delivering high standard of specialist nursing care within the specialist colorectal cancer stoma care service and support the Lead Colorectal Clinical Nurse Specialist and the wider nursing team in ensuring high standards of nursing care provided to all patients receiving care in the speciality.
The post holder will also be responsible for providing professional advice to members of the multidisciplinary team, assist in developing the skills and knowledge of staff to deliver high quality care and achieve the performance targets for the speciality.
The post is based at the University Hospital of North Midlands.
Duties will be across both the Royal Stoke University Hospital and County Hospital sites Main duties of the job Job Role summary The aim of the post is to provide expect care, advice and support patients undergoing surgery for colorectal disease, including the formation of a stoma.
This will include holistic patient assessment, psychological care, monitoring of treatment programmes as part of the colorectal multidisciplinary team.
The post holder is expected to participate in the development of specific colorectal/stoma training and education and clinical supervision of other staff and students.
To provide a continuous supportive service to patient as part of the colorectal nursing team.
This will include liaising with other healthcare professionals, including the Primary Healthcare team in order to provide the best possible care for their patients and carers and family, ensuring continuity of care from diagnosis to community follow ups and the whole patient journey.
To utilise holistic needs assessment and identify patients who might need benefit from referral to other professionals.
To work closely with patient support groups and to liaise as necessary with other teams to ensure seamless care, facilitating open communication and coordination of care.
The post holder acts as a colorectal cancer care resource, collaborated with all members of the multidisciplinary team.
Providing specific nurse acknowledge and expertise on all aspects of colorectal cancer care under the direction of the lead Clinical Nurse Specialist.
