Job summary Are you a registered paediatric nurse looking for a positive career change?A new and exciting opportunity has arisen within the Community Childrens Nursing Service.We are looking for passionate and motivated Children's Nurses who have an interest in working within the community, supporting the provision of care for children and young people within their own homes preventing regular hospital attendances.The role will work closely with partners in acute trusts and primary care networks as part of an integrated health offer for children and young people.
The care will be provided in children's homes, schools, nurseries and across other community settings.The CCN Teams work closely together and support each other as needed to deliver high quality care to patients.
This roles entail a high level of autonomy, independent caseload working and clinical decision making.
You will be supported with regular supervisions, 1:1s and appraisals.

Main duties of the job *To support the Team Leader by carrying out specific health care assessments and/or interventions*To carry out initial assessment and triage of patients with short term health care needs by telephone, at clinical settings or in patient's homes under the supervision of a Team Leader*To provide continuity of care for children on caseload and maintain contemporaneous records accordingly*To support the provision of Community Children's Nursing service within special schools as service needs require, under the supervision of a Special School Children's Nurse.
*May carry out initial home assessments with guidance from the Team Leader, to plan, implement and evaluate programmes of care.
*To work in partnership with families and the multi-disciplinary team to provide care in appropriate settings to maximise life opportunities for the child or young person.
*Report any changes in the child's condition to the Team Leader and medical colleagues and ensure all records are maintained in accordance with Trust policies.
*Participate in the provision, storage and administration of medicines including intravenous therapy in accordance with Service/Trust guidelines and policies and in partnership with pharmacy colleagues*To participate in MDT meetings to support children with complex health care needs.
