Job summary Colchester Medical Practice has an opportunity for a care co-ordinator to join our team on a full-time basis for 37 hours per week.
The post-holder will provide extra time, capacity and expertise to our practice team to help us to improve the care provided to our patients.If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.
Main duties of the job Working with GPs and other professionals within the practice to identify and manage a caseload of patients.
Particularly working with vulnerable patient groups with long-term conditions that may require this extra support.
This may include working with individuals, their carers, families and healthcare teams.
Using in-depth knowledge of locally available support and services todevelop care plans with patient, focusing on prevention.
Supporting the coordination of all key activity, including access to services, advice and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred.
Helping patients prepare for, or follow-up after, clinical conversations.
Supporting digital initiatives.
Using clinical systems on a daily basis, with confidence to run searches and keep care records up-to-date.

Person Specification Experience Essential Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups.
Familiar with local resources and services, including how to access them.
Motivated by helping people, with care and empathy.
Able to deliver person-centred support in a non-judgmental way.
Experience of completing holistic person-centred care planning assessments.
Strong listening and communication skills.
Desirable Diploma Level 2 in Health and Social Care, or an equivalent qualification (desirable not essential).
Primary care experience preferred.
