Job summary The Palliative Care Clinical Practitioner (PCCP) will support delivery of palliative and end of life care, acting as the generalist 'expert' for the locality and working collaboratively across allcommunity services to lead and influence continuous improvement in palliative and end of lifecare.
The post holder will support teams deliver the 'home first' approach, working in partnership with discharge teams/hubs across acute and community services, EICTs and specialist teams.

We are looking to recruit an innovative and motivated senior nurse who is passionate about delivering high quality palliative and end of life care for our patients and their families.
Job description Job responsibilities With a focus on clinical leadership and quality improvement, the post holder will: Support community teams to deliver evidence based, pro-active individualised care; acting as a role model and enabling staff to develop confidence and competence in all aspects of generalist palliative and end of life careIdentify training needs within community teams and facilitate local learningLead on the implementation and ongoing review of local quality improvement initiativesWork in collaboration with our system partners Hospices and acute Trusts to support development, implementation and ongoing review of system wide discharge pathways relating to palliative and end of life care The successful candidate will be a confident leader, who can influence by demonstrating excellent clinical, communication and leadership skills.
They will need to work autonomously and build strong relationships across a wide range of services and teams.Independent Non-Medical Prescribing is an essential requirement of this role and applicants must have completed this course (V300)The role will involve direct clinical care but the post holder will not hold a clinical case load.
