Job summary An exciting opportunity has arisen for a full time Band 6 Primary Care Mental Health Practitioner to join our the East Neighbourhood Mental Health Team for 6 months maternity cover on a fixed term contract.Within BSMHFT our community mental health services are well-established and as part of the BSOL community transformation programme and this is an exciting opportunity to be at the heart of national change and developments in the delivery of community mental health care.We are looking for an enthusiastic and dynamic individuals who is able to work within a multi- disciplinary team, working alongside GP's and other primary care professionals who strive to achieve the best outcomes for individuals within the local community.You will be responsible for providing mental health triage, assessment and outcomes based intervention to meet service users needs within the Primary Care Networks across East Birmingham.
You will be responsible for identifying need and best course of advice, support or intervention, promoting psychosocial wellbeing by identifying and then supporting pathways for service users and carers.
