Job summary This is a fantastic opportunity to join the energetic, innovative Complex Discharge Team at Good Hope Hospital.
It is an exciting time for Complex Discharge with many new pathways and initiatives being introduced and partnerships with external agencies being forged by the complex will provide clinical expertise to support safe, effective and timely discharge working directly with internal and external multidisciplinary teams as required.This post will offer you the opportunity to develop and flourish whilst learning about the operational management of the Trust.
You will be part of a developing team that strives to ensure all patients are given equal opportunity to return home.You'll need to be enthusiastic and have a strong sense of purpose and good diplomatic skills in engaging staff.

Main duties of the job Act as key-worker to co-ordinate individual complex dischargesAct as Trust experts in discharge planning, assisting and supporting the MDT to drive and co-ordinate discharges, ensuring effective, timely intervention from all disciplinesAssist wards and teams with understanding and using the 'Discharge to Assess' modelComplete Health Needs Assessments to facilitate dischargeComplete Fast Track Assessments for palliative patientsProvide education in discharge planning to members of the MDT or other professional groups requiring this teachingSupport transfers to NHS funded community venues/servicesMonitor performance associated with discharge, identifying gaps in service provision that may have a detrimental effect on length of stay, delayed transfers of care or patient experienceWork with multi-agency representatives to reduce length of stay and delayed transfers of care, to achieve performance targets related to dischargeEssential skillsEvidence of knowledge of the Hospital Discharge and Community policyEvidence of knowledge of Discharge to Assess and the hospital discharge pathways.Ability to demonstrate the importance of, and commitment to integrated workingQualificationsEssentialRegistered NurseDesirableDegree or equivalentExperienceEssentialProven post registration experience at ward levelSuccess in working collaboratively across professions and servicesAcute/complex care experienceWorking in a hospital environment About us We are recognised as one of the leading NHS Foundation Trusts in the UK.

Person Specification Qualifications Essential * Registered Nurse on the NMC register.
* Maintained relevant continuing professional development in the past 3 years which supports revalidation.
* Evidence of knowledge of the Hospital Discharge and Community policy * Evidence of knowledge of Discharge to Assess and the hospital discharge pathways.
* Ability to demonstrate the importance of, and commitment to integrated working Experience Essential * Ability to act on own initiative with evidence of sound decision making abilities * Ability to demonstrate up to date experience and depth of knowledge and clinical competence.
* Experience of MDT working * Experience in an acute hospital/community setting.
