We are looking for a dynamic, enthusiastic, and flexible Rehabilitation Technician to work within the Falls Services.The service is a specialist community-based team, including home based rehab comprising occupational therapists, physiotherapists, rehabilitation technicians and a specialist nurse.
This service is aimed at adults aged 60 and above who have fallen or experienced a fracture of hip, wrist or vertebrae in the past 12 months.We are looking for a dynamic, organised, and enthusiastic individual to join our established multidisciplinary Falls and Fracture Prevention Service based at Hodgehill Primary care centre.
Successful candidate will work alongside, Occupational Therapists and Physiotherapists, Nurse supported by other rehabilitation Technician and administrators.
Main duties of the job As part of the integrated service provision the post-holder will work within the Falls and Fracture Prevention Team to assist in the prevention of inappropriate hospital admissions, the facilitation of hospital discharge and the provision of home-based treatment in line with the overall objectives of the service.
The post holder will ensure access to urgent and routine therapeutic intervention, provide and monitor treatment programmes at home and ensure referral to long term health and social care interventions.
They will work in partnership with patients, carers and other health and social care agencies to promote independence and develop individual plans of care relevant to patient need.
You will be expected to work multi professionally, support other team members' roles and contribute to the future development of the service.
This is a community post so a car driver and access to a vehicle are essential requirements.
The role provides an exciting challenge, supported by comprehensive supervision and training.
The successful candidate will require good written and verbal communication skills and be computer literate.
We also hold good links with the university to support students who are on placement with return, we will provide opportunities to work with a variety of complex and noncomplex conditions.
Support with learning and development, regular supervision, and appraisal, and participate in the In-service training programme.
