Job summary LOCATION & FUNCTIONThe role will be hybrid working, to deliver a Workand Health Coaching, which can include working in the GP surgeries withinLeicester, Leicestershire & Rutland This is an exciting opportunity to work in a brand newrole within Primary Care General Practice, working the practices withinLeicester, Leicestershire & Rutland (LLR)Primary Care Newtorks (PCN) The role will give you a uniquechance to work as a member of the Care Navigation Team, whilst also buildinglinks with a range of other local services and be part of a county wide Workand Health coach network.The successful candidate will be part of the WorkWellprogramme and will work with people from a range of backgrounds to help them toremain, thrive in work and return to work.
They will work closely withparticipants within the local community to assess the barriers they experiencein returning to work and provide support and coaching along with health andemployment advice to overcome these barriers and return to workThis role would suit someone with previousexperience of coaching and mentoring.
Clinical experience is not required, andtraining will be provided for the successful candidate.Please note that numerous vacancies will beadvertised for this Work and Health coach role, in other Primary Care Networks(PCNs) and federations across LLR.
Main duties of the job To manage acaseload of clients who have experienced common mental,physical and social health problems or long term conditionsTo carry out avocational assessment, to identify vocational needs and develop actionplans in agreement with the individual.Ensuring that aParticipants eligibility is checked as outlined in the WorkWell GrantGuidance.To provide anevidence based, person-centred and holistic initial assessment ofbarriers.To provide alight-touch holistic support for health-related barriers to employment.To provide aprovision of a single joined-up view and pathway into local services totackle specific need.To ensure multiplereferral pathways into and onwards from WorkWell servicesacross the range of partners to ensure maximum opportunities for people toengage in using WorkWell services.To provide allinformation/documentation requested by DWP for the purposes ofactivities undertaken by DWP to evaluate the effectiveness and success of WorkWell.
About us Ripple Touch provides workand health coaching services to GP practices within the Primary Care Networks,to help support patients in the wider community.The Work and Health coach will conduct assessments toidentify the key health barriers to employment and provide health andemployment advice to overcome those barriers.They will manage a caseload of people whose barriers to employment may bebecause of mental health, long term conditions and Musculoskeletal conditions,that have received a Fit Note by a healthcare professional.
Using the WorkWell assessment plan, the postholder will provide 1:1 coaching and prescribe either multi-disciplinaryin-house support or signpost and refer to external services.
Job description Job responsibilities Key responsibilities will include:To manage a caseload of clients who have experienced common mental, physical and social health problems or long term conditions.To carry out a vocational assessment, to identify vocational needs and develop action plans in agreement with the individual.Ensuring that a Participants eligibility is checked as outlined in the WorkWell Grant Guidance.To provide an evidence based, person-centred and holistic initial assessment of barriers.To provide a light-touch holistic support for health-related barriers to employment.To provide a provision of a single joined-up view and pathway into local services to tackle specific need.To ensure multiple referral pathways into and onwards from WorkWell services across the range of partners to ensure maximum opportunities for people to engage in using WorkWell services.To provide all information/documentation requested by DWP for the purposes of activities undertaken by DWP to evaluate the effectiveness and success of WorkWell.To maintain detailed and accurate case notes, action plans and WorkWell participant paperwork and upload these on to the management information system within the allocated timeframe.To provide information, advice guidance & support to individuals about the pathways of opportunities available to them to gain, return and sustain work.To support individuals to gain accurate benefits advice, complete paid job applications, compile CVs and to practically prepare for interviews.To liaise and arrange meetings with health professionals, partner agencies (voluntary and statutory sectors) and/or employers concerning the individuals needs, including partner organisations that work with diverse groups to develop joint working that will enhance service delivery and increase opportunities for individuals.To deliver training and workshops (as required) to clients on job applications, CV writing and interview skills to increase their chances of moving into employment.To take part in informal training with colleagues from external organisations to help raise awareness and improve their knowledge about this pilot and the WorkWell model, as well as supporting them to understand the common barriers and LTCs.To ensure that the service eligibility is adhered to, providing information on alternative employment support services for those who are not eligible, and monitoring progress and outcomes of those supported by WorkWell ensuring that stringent monitoring deadlines are adhered to, and KPI evidence is gained regularly.To liaise and communicate regularly with the referrer and other relevant healthcare staff to ensure that all parties are kept informed and cooperate in helping the individual achieve, return and sustain employment.To collect and collate statistics that supports the ongoing monitoring of service delivery/ quality.
To provide information for service reports as required and participate in regular audits and evaluations.To work as part of a team to further develop quality standards to continue to improve service delivery and the implementation of WorkWell.To offer advice and guidance to the Administrator and training and support to new team members.To work flexible hours as required to meet the needs of the individuals and the vocational serviceAlong with your main duties, you will also be expected to carry out any other duties that are reasonably asked of you.
