Your duties will include:· Answering phone calls and documenting key information in the Care Planner system.· Managing care call cancellations and resolving all PASS alerts promptly.· Conducting audits to ensure client files are compliant and up-to-date.· Running reports and allocating reviews or new business to team leaders.· Maintaining accurate and current client care plans and medication records.· Coordinating new care packages to ensure seamless service initiation.· Managing spreadsheets to track new and closed care packages.· Completing important documentation, such as LPOAs, GP summaries, and impact risk assessments.· Supervising and ensuring the proper management of client medications.· Collaborating with healthcare professionals, including Nurse Practitioners, GPs, and Pharmacists.· Supporting team leaders in organising client assessments.· Assisting the senior management team with audits of service users' records.· Promptly escalating complaints or issues to the Deputy Manager for resolution.About Horizon HomecareAt Horizon Homecare, we believe in creating a supportive and rewarding environment for both our clients and our staff.
