Job summary We are looking for enthusiastic andmotivated Nurses, Allied Health Practitioners and Paramedic Practitioners tojoin our highly regarded Urgent Community Response Service,based at Woking Community Hospital.We provide 24-hour responsive NHS community services to patients across NorthWest Surrey in conjunction with the wider Integrated Care System.Working as a member of the multi-disciplinaryteam you will undertake assessments, clinical diagnosis and treatment ofpatients who may require management and/or rehabilitation interventions intheir own environment to prevent acute hospital admissions and to supportcomplex hospital discharges.
Although expected to work autonomouslyyou will not be alone.
The team currently includes Physiotherapists,Occupational therapists, Nurses, paramedics and Rehabilitation Assistants and we workclosely with colleagues from primary care, locality health hubs, adult socialcare and other community services.

Main duties of the job To work closely with the UCR AdvancedClinical Practitioners(ACPs) & Clinical Leads for frailty, CommunityNursing, Frailty GPs, Adult Social Care, community services and the thirdsector to provide fast reactive services for patients with decompensatedfrailty and ensure rapid delivery of treatment, care planning to support acutehospital admission avoidance where appropriate with a focus on the 9 CommonCritical Conditions- Falls; Decompensation of Frailty; Reduced Function/Decondition/reduced mobility;Urgent equipment provision, Confusion / Delirium, Palliative / EOL crisissupport; Urgent Catheter Care, Urgent support for diabetes; Unpaid Carer breakdown.
[To provide advanced assessment and care planning, including historytaking and physical assessment for patients with frailty.To provide strong holistic assessment and treatment planning of patientswith frailty, without direct supervisionTo work in conjunction with a wide range of clinical colleagues and specifically,primary care and community teams and Social Care professionals, leading andfacilitating a patient or client focused, co-ordinated case management approachacross primary and secondary care for people who are most vulnerable to, and athigh risk of repeat admission to hospital.Note: we do not offer UKVI sponsorship.

Job description Job responsibilities The Urgent Community Response (UCR) Team aspireto provide a 24-hour/7-day responsive NHS community service.The Band 6 UCR Practitioner can be a Nurse, Allied Health Practitioner orParamedic.
There is the flexibility within the multidisciplinary team to workacross days, nights and to do internal rotation and you are able to workautonomously, managing patient assessments within the specialty whilst workingas part of the larger multidisciplinary team, delivering individualised andpersonalised direct patient care to patients across North West Surrey inconjunction with the wider IntegratedCare System.
Theteams are commissioned to reflect the needs of the local community.
Serviceaims include urgent case management and hospital admission avoidance, where thefocus of the role is to lead the identification and clinical assessment ofpatients who will benefit from advanced complex hospital discharge care, or urgentresponsive admission avoidance using the Virtual Ward if required, with care provided in their ownhome by the multi-disciplinary team.Please see attached Job description for further details Person Specification Skills Essential Excellent communication and interpersonal skills Broad range of enhanced clinical skills Ability to advocate patient issues Ability to demonstrate leadership skills Demonstrates organisational skills: including the ability to make decisions and to prioritise Ability to understand and interpret research findings/evidence-based care and apply to practice.
Good knowledge of health and safety and risk management Computer literate including emails and spreadsheets.
Effective written and verbal communication skills Experience Essential A minimum of two years post registration experience Experience of caseload management including responsibility for complex care packages for vulnerable people Experience, underpinned by knowledge of working with and understanding the complex needs of patients in a primary care/community setting Involved in the implementation and management of change Evidence of innovative practice Experience of initiating or participating in clinical audit/research relating to clinical practice Participates in regular clinical supervision Experience of working with long-term conditions and frailty Qualifications Essential Registered General Nurse/ Registered Mental Health Nurse/Practitioner or BSc/Diploma leading to inclusion on the Health and Care Professions Council Register as an Occupational Therapist, Physiotherapist or Paramedic Post registration qualification or University Degree Teaching and assessing qualification Mentorship or equivalent mentorship qualification PETALS Desirable Masters degree or equivalent experience gained by undertaking on-going personal development and training Management/Leadership Qualification/ development programme
