Job summary In line with our clinical strategy we have developed an Elderly Assessment Unit at Wansbeck Hospital to which supports the move to alternative pathways for urgent frailty assessment and redirects patients to the most appropriate environment for their assessment and treatment resulting in more people remaining within their home environments.
This is achieved through the following: o Same day direct access pathway for NEAS and Community services including Primary careo Virtual ward - step up facility to assess patients of concern in the community, step down facility to enable positive risk taking for patients in the emergency department and NSECH wardso 2 hour response - step up facility to assess patients of concern to community teamso Single point of contact for urgent advice for frail patients with aim of utilising urgent assessment services and community teams rather than NSECH emergency department if appropriateo Referrals from GP/Community Services within 2 week wait providing a semi urgent response.
Main duties of the job This post is a Trust appointment and will be subject to the Terms and Conditions of Medical & Dental Staff (see as amended from time to time.
