Job summary As a Clinical Pharmacist you will work with our current Clinical Pharmacist Team to further develop and manage medicines optimisation services.You will work in the Trent PCN practice's, and therefore have the opportunity to build up relationships with the primary care team.You will work together to manage repeat prescription authorisation and reauthorisation, acute prescription requests, management of transfer of care and systems for safer prescribing.You will have the opportunity to develop areas of clinical interest in a patient facing role, develop or improve your prescribing skills and improve the management of patients with long term conditions.
Mentoring and support will be provided.You will be supported through your CPPE pathway and also your IP course if needed by our team of Pharmacists.You will not be working in any practice alone.
Main duties of the job Youwill provide primary support to general practice staff with regards toprescription and medication queries.
You will help support the repeatprescription system, deal with acute prescription requests, and medicinesreconciliation on transfer of care and systems for safer prescribing, providingexpertise in clinical medicines advice while addressing both public and socialcare needs of patient in the GP practice(s).Youwill provide clinical leadership on medicines optimisation and qualityimprovement and manage some aspects of the quality and outcomes framework andenhanced services.Youwill ensure that the practice integrates with community and hospital pharmacyto help utilise skill mix, improve patient outcomes, ensure better access tohealthcare and help manage workload.
The role is pivotal to improving thequality of care and operational efficiencies so requires motivation and passionto deliver excellent service within general practice.Youwill work with the other CPs in our large team being fully supportedthroughout.
About us K2 Healthcare is a GP Federation covering 16 practices:10in Grantham and surrounding villages and 6 in Sleaford.
We cover 130,000+patients across all sites.
We have a team of 12 CPs working for us including 3senior CPs and 6 pharmacy technicians.
We are also working with Trent Care Network, covering fivepractices with a population of 40,000 to provide support and mentorship fortheir Clinical Pharmacists as they grow their team.
This post will be workingout of the 5 sites and will have the support of all the K2 Team.Trent PCN have invested heavily in their clinicalpharmacist programme.
The CP Team are all atdifferent stages of completing their CPPE / IP course and are happy to speak toyou regards their experience with K2 Healthcare.
