Job summary We are seeking an experienced RN Child Nurse to driveforward quality focused care for young people who are moving from thechildrens department to adult specialities within Worcestershire AcuteHospitals NHS Trust and / or primary care.The post holder will be involved in transition planning (moving to adulthood)across the Trust.
The focus of this role will be to empower, educate and informpatients and their families through the transitional phase in order that theymay continue their care in an adult environment.
The post holder will provideinspirational leadership and direction to all members of the team and service.Excellent communication & leadership skills arerequired, along with the ability to work independently and within themulti-disciplinary services.
A passion and interest in adolescent health andtransitional care is essential.
Main duties of the job Make plans with the young person and work directly with families.
Support those working directly with families by ensuring oversight of processes to enable better outcomes, better experience and better use of resources.

Job description Job responsibilities JobSummaryThe post holder will be part of the Childrensmulti-disciplinary team and will be professionally accountable fordemonstrating visible and accessible leadership and Trust values.The post holder will support the PaediatricMulti-disciplinary Team and adult service team to ensure seamless Transition isembedded within the Trust.The post holder will work as a highlyspecialised nurse co-ordinating with children and young people and the relevantservice.
This includes facilitatinginput from supportive services such as Learning Disability or Neurodivergence.The post holder is expected to exercise a highdegree of professional autonomy tomanage caseload and make critical decisions to satisfy the expectationsand demands of the role.The post holder will support the ChildrensClinical Audit Programme promoting quality improvement initiatives.Act independently within the paediatric teamguided by best practice and Trust policies and guidelines.To act as a carer and advocate for familiesgiving expert support and advice based on best practice and close communicationwith the paediatric and relevant adult service.To act as a nursing expert in transition bothlocally and regionally.PRINCIPLE DUTIES AND RESPONSIBILITIESClinical ResponsibilitiesProvide a clinical-led transitional service as aspecialised co-ordinator working with a specified group of older children/youngadults.To act as an expert resource, giving advice,education/training and support that is appropriately planned to meet theindividual's needs empowering them to manage their treatment regimes.The post holder will be key at identifyingpotential development opportunities within the transition pathway and workingwith the Paediatric leadership team to implement these.To project lead and implement change andpathways in partnership and collaboration with other Health Care Professionals.To work within local and national frameworks toimplement service changesTo lead on the running of a support group fortransitional patients and their family/carers.CommunicationMaintain an effectivecommunication network with all multi-professionals, facilitating amultidisciplinary approach to benefit the children, young people and theirfamilies.To work witheach family individually assessing the level of support they will need usinginterpersonal skills to build a trusting relationship thus promotingconcordance, recognising the dynamics of change for each family.To take alead in developing the quality and availability of transition relatedinformation for young people and their family units.Act as anexpert resource for medical staff both within the trust and outside.Leadership and Management In conjunction with the other multidisciplinary colleagues, contributemonthly/quarterly reports of case mix, workload and education activitywhichwill be presented at Directorate meetings.Representingand reporting to Trust committees, such as Childrens and Young Persons Board.
To participatein the presentation and publication of relevant research and best practiceinitiatives.To be activelyinvolved with conferences and meetings, developing presenting skills in theregional and national setting.
To represent the Trust nationally.To networkwith other Transition Nurse Specialists throughout the UK, joining appropriateorganisations in order to collaborate, share and benchmark practices andinitiatives.To obtainevaluate and disseminate information to facilitate best practice for children,young people and their families moving between paediatric and adult services.Undertake regularmeetings with group and nurse manager to ensure co-ordination of care,continuity and planning changing in frequency from weekly to monthly accordingto current service needs.Maintainconfidentiality of patient/family and staff information.
Be discerning whenconfidential information should be shared.To undertakeall expanded role procedures according to agreed protocols within the scope ofProfessional Practice and identify areas for further advancement.To develop andmaintain own clinical skills and knowledge of the transition process that isevidenced based aiming to achieve a 'best practice' principle, keeping up todate with new advances in treatment and new developments in Transition.
Manage timeoff and delegate care of patients appropriately.Professional ResponsibilitiesMaintainown professional knowledge development and competence by keeping up to datewith advances and research pertaining to Transition.Beaware of the Code of Professional Conduct in the performance of the advancednursing and be professionally accountable for own actions during the workingshift.
To work autonomously,when required, having accountability for decisions made (in accordance with theNMC Code of Professional Conduct).To manage owncaseload managing own time on a weekly and monthly basis.To recognisethe level of emotional stress encountered when with patients and their familieswho are dealing with life threatening and control issues and to seekappropriate personal support as required.Education and TrainingTo act as arole model and clinical expert in speciality when liaising with the localprimary healthcare teams and other relevant community health professionals inorder to promote a better and on-going understanding of each child / youngpersons condition and associated symptoms and treatment regimes to promoteeffective continuity of care.Participate ineducation and practice development on a Trust wide basis and as required in anational level by attending, presenting at, and/or assisting with theco-ordination of study days, seminars, forums and conferences.
To encourage other staff to recognise theneed for nursing research.Be an expertin transition and regularly undertake active teaching and assessing in theeducation and training of clinical and non-clinical staff to support theeducation strategy.Assess owneducational needs and take steps to keep up to date with clinical and nursing developmentsin line with requirements of the Nursing & Midwifery Council.Clinical GovernanceThepost holder will support effective governance by disseminating good practiceand promoting clinical audit and use of evidence based multi-professionalstandards.Toparticipate in all aspects of Clinical Governance and Risk Management includingparticipating in the development and implementation of audit and guidelines.To participatepromptly and effectively with the investigation of complaints and incidents inline with Trust policy and procedure.To maintain high standards ofnursing care within own practice and monitor the standard care for the child oryoung person requiring admission in collaboration with senior ward nursingstaff.Service DevelopmentTo ensure theconstant provision of effective, high quality nursing care to all patients movingfrom children to adult services.To take a lead in developing thequality and availability of transition related information for families andcommunity health care professionals.To lead on therunning of a support group for transitional patients and their family/carers.To be activelyinvolved in the development of Transition related protocols and procedures care pathway, in conjunction with other members of the paediatric and adult multidisciplinaryteam.To participateand be actively involved in multidisciplinary research studies of relevance tothe care of young people moving from children to adult services, playing a leadrole in liaising between the patient and the research team.

Must have the ability to travel between sites Skills and Knowledge Essential Demonstrate a knowledge of duties required for the post Well-developed analytical and judgement skills Knowledge of Transition to adult services frameworks, such as Ready, Steady, Go Well-developed written and verbal communication skills, both verbally and written at various levels; with ability to adapt communication as required for children, young people, and families under stress Knowledge and understanding of relevant legislation /guidance and initiatives that influence clinical practice and patient group.
Ability to motivate others and be self-motivating, to develop self and other practitioners Able to make skilled and balanced decision which will affect families, staff, and service provision Desirable Knowledge in the use of databases Ability to produce and present clear reports Excellent negotiation skills and conflict resolution skills Provide supervision for staff including clinical, managerial, and professional Evidence of completing the audit process, producing outcome, sharing learning and action plan.
Demonstrate an understanding of the additional requirements to accommodate young people with learning disabilities and / or neurodivergences.
Qualifications Essential Degree or Diploma Level Education as Registered Nurse (RN Child or RSCN) Current registration with NMC Teaching and Mentoring qualification Evidence of continuing professional development (CPD) within the last 12 months Desirable Possesses or willing to work towards undertaking a masters degree Teaching qualification Specialist training in Transition to Adult Services, speciality training Leadership qualification Experience Essential Substantial/ Significant post registration experience, clinical and managerial in relevant areas of practice Clinical experience in the care and management of children /young people Experience of supervising staff and students and learners in practice Organisational skills and ability to lead a team and manage performance Evidence of formal and informal teaching skills Evidence of working independently to meet deadlines Desirable Project management Experience in developing and implementing policies & procedures Research and Audit and benchmarking and service evaluation work Experience in writing and presenting reports Previous experience of working as a clinical specialist nurse Experience at Band 7 Personal Qualities Essential Pleasant and calm disposition Able to work independently Ability to adapt to the changing needs of service Able to lead and motivate others Self-confident, reliable, with a good attendance record.
