Job summary We are looking for a forward thinking individual to join our Commissioning Team for Mental Health and Learning Disabilities.
The post holder will ensure the provision of seamless, integrated services or package of care for individuals who meet continuing healthcare criteria or are subject to 117 that require commissioned services that cannot be met by core services.The Transition Case Manager acts as a link person for people with complex needs who are approaching transition to adulthood.
They will have a key role in the implementation of the National Framework for Continuing Healthcare within the mental health and learning disabilities division and will provide a case management function to a caseload of patients / service users eligible for CHC.The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job The Transition Case Manager is a crucial role in the pathway for those individuals who are transitioning into adult services.
They are responsible for gathering information for individuals to gain an understanding of their needs and will work collaboratively with other disciplines, and along side care co-ordinators to ensure the individual has the appropriate care during their transition.
They will have an understanding of The Transition and Handover Guidance (WAG).
The Transition Case Manager has strong links to CMHT's and CAHMS ensuring that the relevant professionals are involved.

Person Specification Qualifications Essential Professional Registered qualification RMN, RNLD, DipCOT or Social Worker Desirable Teaching/Mentoring qualification.
Experience Essential Experience of working in a professional capacity within the field of Mental Health or Learning Disabilities Desirable Experience of working with children and young people who have serious mental illness and/or complex needs and their families and carers in the community.
