Job summary We are looking for an enthusiastic dedicated qualified Advanced Clinical Practitioner or a person working towards this, who is keen to work within our Frailty (SDEC) Same Day Emergency Care.
You will be working in the Frailty Directorate, within Medicine Division, at Royal Sussex County Hospital, an acute hospital in a care of the elderly setting.
This service is focused on delivering rapid assessment; bespoke care to our elderly patients and provide treatments and provision to prevent prolonged stay in hospital and unnecessary admission.We are looking for individuals that will work effectively and efficiently: we place a strong emphasis on continuous quality improvement, acting as an agent in change in achieving a culture that is flexible andparticipates positively in improving patient outcomes,and ensuring that we consistently meet and exceed the highest standards of care.As a ACP/tACP you will be expected to reflect on the four pillars of advanced practice: Clinical Practice, Leadership, Education of self and others, and Research & development.The role is varied, equally challenging and rewarding, where you will have the opportunity to really make a difference to the experience of our patients.We welcome applications from those who currently hold a UK NMC / HCPC registration.

Main duties of the job The post holder will work in the Frailty Same Day Emergency Care Unit based on C8 in the Louisa Martindale Building at the Royal Sussex County Hospital.
You will be expected to work collaboratively with multi-professional colleagues within the service to deliver high-quality care for our local population accessing Frailty SDEC including Comprehensive Geriatric Assessment.You will be responsible for the initial assessment and management of patients, as well as organising their initial investigations and formulating a management plan.You will work collaboratively to conduct audits, collect data and have a key role in service development.As a relatively new service, this service is currently operating on a 5-day service Monday-Friday, but successful candidates will be expected to enter into a 7-day rota as the service develops.

Job description Job responsibilities Please review the attached Job Description and Person Specification.The roles will work collaboratively, within multi professional teams.
The qualified ACP will be expected to make complex, autonomous decisions utilising expert practice and knowledge within a supportive team.
The trainee roles will be supported while they undertake the academic study and training required to develop into the role.The qualified ACP (band 8a) will have a registered MSc in Advanced Clinical Practice or have completed a PG Cert & Dip in Advanced Clinical Practice and is undertaking their dissertation/learning improvement programme.
They will be an established Non-Medical Prescriber (NMP) and able to function as an autonomous practitioner across the surgical clinical areas.
The job plan has 10% non-clinical protected time to achieve all four pillars of advanced practice.The trainee ACP (band 7) will be willing to undertake, or is currently registered on an MSc in Advanced Clinical Practice.
Consideration will be given to applicants who have completed the physical assessment module, are registered non-medical prescribers, or are completing the NMP course.
They will demonstrate a commitment to completing an MSc in Advanced Practice.
Funding and 20% non-clinical time will be provided to undertake the MSc.If you are enthusiastic, dynamic, highly motivated and committed to delivering high quality care then we would like to hear from you.
Applications are welcome from Nursing and Allied Health Professionals with HCPC/NMC registration that are eligible to under non-medical prescribing in accordance with their professional body.
Person Specification Experience / Qualifications Essential HCPC/NMC qualification MSc in, or working towards: Advanced Clinical Practice Experience in band 7 or above Clear and demonstrable evidence of competent practice in the management of patients.
Desirable Independent prescriber Teaching experience/qualification Training in change management and/or clinical service improvement.
IRMER (Imaging Referral by Non-Medical Professionals) certification Equality, Diversity, and Inclusion Essential Evidence of having championed diversity in previous roles (as appropriate to role).
Evidence of having undertaken own development to improve understanding of equalities issues
