Job summary An exciting opportunity has arisen within the Health and Justice service for the role of Reconnect Support, Time, Recovery Worker.
Reconnect services identify people who have substance misuse, mental health, physical health, learning disability, neurodiversity or other vulnerabilities and what support they require on release from prison or Immigration Removal Centre.
We will start to work with people up to 12 weeks prior to release.
We look to identify unmet need and vulnerabilities that often lead to or exacerbate offending behaviour, offer onward referral and signposting as appropriate.
We liaise across agencies to support onward planning and intervention to support people to meet their health needs and reduce health inequalities.
We offer support until the health needs are met or up to 6 months post release depending on which comes first.
A key feature of the STR worker role within Reconnect is building a rapport with someone prior to their release so within the role it will be required that you are able to and willing to attend the prisons within our catchment area - HMP/YOI Brinsford, HMP Dovegate, HMP Drake Hall, HMP Featherstone, HMP Oakwood, HMP Stafford, HMP/YOI Swinfen Hall, YOI Werrington.
The service works with people from the age of 18 however due to potential cross-cover there may be a need to work with those under the age of 18 when required.
*THIS ROLE IS SUBJECT TO PRISON VETTING**The post holder will be expected to travel across the MPFT footprint* Main duties of the job **Please refer to the attached Job Description in summary:** Working with service users referred to Health & Justice and the individual's carers, helping to promote self-determination, choice and autonomy.
To effectively establish a therapeutic relationship with service users and relatives / carer.
Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
Assess patient's individual care needs, agree and refer on to appropriate agency.
Involve the individual in the planning and prioritisation of their activity where possible.
Provide short term intervention to encourage individual to engage with services and attending their initial appointments.
Complete and contribute, where appropriate to robust risk assessments and risk management plans.
Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
Utilise agreed outcome measures to review the effectiveness of process, as part of Health & Justice Service.
Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
