Job summary The stop smoking practitioner is responsible forsupporting and facilitating the delivery of smoking cessation programmes withinthe Healthy Communities integrated lifestyle service for people in WarwickshireWe have opportunities for new practitioners acrossWarwickshire, Clinics run from Monday Saturday 20:00 hoursFull and Part time roles availableA full driving license and business insurance isrequired due to the nature of the roleYou will receive your own cohort of patients and Faceto Face clinics including the delivery of group sessions to support and maintainand will help to improve the accessibility of Warwickshires Stop SmokingService through the pro-active promotion of the smoking cessation programme andreferral pathways working with key health care providers, allied professionals,community events, local agencies, and the general public.This can be a challenging but extremely rewardingrole for the right candidate.
You will be part of both the smoking cessationteam as well as the wider service team.
You will also receive additional in-housetraining including data security, first aid and safeguarding.

We would liketo hear from you if youHave strong interpersonal and communication skills.Possess a minimum of 1 year experience working in a community-based smokingcessation service or Healthcare Setting.Are experienced in using behaviour change models within a work context.Can demonstrate a clear understanding of the range and appropriate use of differentsmoking cessation interventions.Display strong knowledge of the harmful effects of smoking, smoking as anaddiction behaviour, withdrawal, and relapse.Are flexible with good time management and planning skills.Are interested inmotivating and supporting both individuals in virtual and community-basedsettings.Can raise publicawareness and provision of consistent healthy lifestyle messaging, on issuesrelevant to the health of the community and in a multi-cultural setting.Have experience in building strong links andrelationships with a diverse range of groups, Organisations, and agencies.

Job description Job responsibilities JOB DESCRIPTIONStop Smoking practitionerMAIN PURPOSEOur Stop Smoking Practitioners deliverone to one and group stop smoking interventions within a wide range ofsettings.
Across Warwickshire, our service willinclude work within one to one and group settings and also involveevening and weekend work.The post holder will be required towork flexibly across the County as required ensuring their skills, expertiseand knowledge are fully utilised.
Thispost willProvide information to smokers to enable themto make an informed choice to make an attempt to stop smoking and then tosupport the smoker with a formal evidence based quit programme.
Develop and improve accessibility of the WarwickshiresHealth Communities Service and Stop Smoking Service through the pro-activepromotion of the smoking cessation programmes and referral pathways amongstprimary and secondary care services, other key health care providers, alliedprofessionals, community development initiatives, local agencies and thegeneral public.
Provideongoing support to a caseload of referrals, supporting each quit attempt inline with national and company standards.Contribute to the planning, delivery anddevelopment of the stop smoking service including specific project work.
This post will alsoIn conjunction with the Delivery team leadplan and deliver an appropriate and accessible cessation service within thedelivery team area.Provide cessation support including one to oneand group support, assessing clients suitability for Stop Smoking treatments.Adhere to service protocols and procedures inrelation to the provision of supportive pharmacotherapy medication.Collate accurate clinical records in line withnational PHE or NCSCT requirements and inputting data into the service web-baseddatabase in a timely and professional manner.To support the service in delivering keyperformance indicators and activity as set out within the servicespecification.Support the delivery of promotion activityacross Warwickshire in line with the service marketing strategy.Routine maintenance and the checking andcalibration of carbon monoxide monitors in line with service schedules toensure validity of quits in line with department of health guidelines.Work flexibly to provide a coordinated serviceacross the localities to meet the needs of the client group.
This will includework in the evening and some work on weekends where required.Maintain up to date clinical knowledge,undertake evidence-based practice and promote the use of new and innovativepractice.In conjunction with delivery team lead,conduct analysis and evaluation of designated programmes in order to improvethe quality of service.
Be responsible for identifying issues orconcerns to line manager, so that appropriate action can be takenContribute to ongoing needs assessment, auditand other governance programmes in relation to activities as part of the healthimprovement agenda.

Ensurefollow up of clients is done in line with service protocol Keepaccurate records in accordance with trust policy and data protection Tomaintain an up to date electronic diary that is available to colleagues withinthe Stop Smoking team To assistin producing reports and analysis of data that contribute towards improvingservice delivery and better client care To utilisea range of complex motivational, influencing and negotiating skills to increaseclients motivation and adherence to stop smoking.
Responsiblefor personally identifiable client data as part of ongoing programme attendanceetc., working within the data protection policies.

Role incorporates evening weekend working, with variable levels of travel across Staffordshire, working in a variety of settings including home visits, the community and community centres.
Full UK Driving licence and access to a car for work- with business purposes insurance.
Ability to travel across Warwickshire to areas where public transport may not be available.
Evidence of a personal commitment to continuing professional development and to maintaining an up-to-date professional knowledge supporting the development of business within the company.
This post involves working with vulnerable adults and as such the post holder will be required to apply for a disclosure of criminal records at an enhanced level DBS.

Experience Essential Minimum of one year experience working in smoking cessation programmes across a range of service settings, including community, primary care and in patient settings.
NCSCT Accredited or Registered Stop Smoking Practitioner Experience of working with adults in a one to one and group environment, to support positive lifestyle change.
Experience of using behaviour change models within a work context Experience of working in a multi-cultural setting with patients of different cultures, genders and ages.
Experience of working to time schedules.
Evidence of experience in embracing and implementing change to provide high quality health outcomes.
Experience of working to challenging performance targets and achieving excellent outcomes.
Desirable Sound knowledge of delivering smoking cessation interventions and advising on smoking cessation to special populations.
Knowledge of NICE guidance, NCSCT and PHE standards relating to the treatment of smoking cessation Knowledge and understanding of the Patient Confidentiality Act, Safeguarding and the Data Protection Act.
Qualifications Essential Education to GCSE Standard at grades A-C for a minimum of five GCSEs or equivalent including Mathematics and English language or literature Desirable NCSCT Smoking Cessation Certified.
Training will be provided however health degree or equivalent is advantageous.
