Ward 52 is a 32 bedded Acute Frailty Unit.

The post holder will support the team inproviding care to patients acting under the direct supervision of the wardmanager / Sister.
The post holder should work effectively within a teamenvironment, have good communication skills, initiative, demonstrate enthusiasmand service improvement as well as sharing our passion and visions at CDDFT.
Main duties of the job MAIN DUTIES AND RESPONSIBILITIES To assess patients needs and develop, implement andevaluate programmes of care.
To organise the delivery of nursing care, deploying staffeffectively.
To be aware of, and communicate to team members, changesto patient information and treatment plans, both verbally and in writing.
To adhere to NMC standards of record keeping.
To assist the Ward Sister in setting, monitoring andmaintaining standards of care.
To participate in nursing audit and service improvementactivities.
To be aware of resource constraints and the importance ofworking within budget.
To provide advice and act as role model to other membersof the team.
To be proactive in managing risk and maintaining patientsafety.
To supervise the work of nursing support staff, ensuringtasks undertaken have been delegated appropriately.
To contribute to theday-to-day management of the Directorate, co-operating with colleagues toprovide an appropriate level of nursing service and to address bed managementissues.
To participate in education and training of junior staff.
To involve patients/carers in the planning anddelivery of care.
